About this file

This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases for use in conjunction with the state's Request for Proposal process, specifically related to MPA 54 Interpreting and Translation Services. The form requires bidders to certify that they and their parent corporations, subsidiaries, affiliates, and subcontractors do not have conflicts of interest with any officials, officers, or agencies involved in the RFP, and that they did not materially participate in or provide consultation regarding the RFP's requirements, technical aspects, or formation. Bidders must also attest that the RFP does not relate to any audit, examination, independent verification, review, or evaluation of the bidder's work, financials, or operations performed on behalf of the State of Rhode Island.

The attestation must be completed and signed by an authorized signatory, with the bidder's entity name, solicitation number and title, date of signature, and typed name of the authorized representative clearly indicated on the form. The document defines "conflict of interest" as any situation where private interests or relationships interfere with the interests of the State, while clarifying that being a current or past vendor does not automatically create a disqualifying conflict. Additionally, the form notes that formal responses to a prior Request for Information do not constitute participation or consultation that would disqualify a vendor from bidding on a subsequent RFP.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
