About this file
This is a Cost and Payments submittal form for the City of Phoenix Parks and Recreation Department's Event Equipment Rental Services solicitation (PKS-IFB-26-0175). The City seeks a contractor to provide equipment rental services across 150 line items including seating, tables, tents, canopies, staging, audio-visual equipment, lighting, power distribution, crowd control accessories, and climate control equipment for various City-owned or City-approved locations. The contractor must guarantee delivery within two working days for non-emergency purchases and within four hours for emergency services at no additional delivery fees. A non-mandatory pre-offer conference is scheduled for January 6, 2026, at 11:00 AM, with a written inquiry deadline of January 9, 2026, and offer submissions due January 26, 2026, at 2:00 PM. The contract term is five years commencing on or about April 1, 2026, and will be awarded to the lowest responsive and responsible bidder meeting all specifications.
Vendors must select one of two payment term options: either a prompt payment discount of the contractor's specified percentage at 30 days or 0% net 45 days, with payment effective after receipt of invoice or final acceptance of products (whichever is later); or immediate payment upon invoice approval through enrollment in the City's Single Use Account (SUA) Program administered by the City's servicing bank, whereby the vendor accepts all merchant bank transaction costs and agrees not to transfer these charges to the City. Contractors must maintain comprehensive insurance coverage including $2,000,000 General Liability, $1,000,000 Automobile Liability, and statutory Workers' Compensation limits. Pricing shall be firm and fixed for the initial one-year period, with annual price increase requests permitted thereafter upon written documentation. Liquidated damages of $2,000 per calendar day apply for delays in equipment setup, takedown, breakdown, and cleanup. The solicitation includes provisions for potential Disadvantaged Business Enterprise participation and requires compliance with nondiscrimination statutes including Title VI of the Civil Rights Act, the Americans with Disabilities Act, and equal employment opportunity provisions.

COSTS AND PAYMENTS
(please complete and return with the submittal)
PAYMENT TERMS & OPTIONS: Vendors must choose an option, if a box is not checked, the
City will default to 0% - net 45 days:
____ Contractor offers a prompt payment discount of either ______% - 30 days or 0% – 45 days - to apply after receipt of invoice or final acceptance of the products (invoice approval), whichever date is later, starts the 30 days. If no prompt payment discount is offered, the default is 0%, net 45 days; effective after receipt of invoice or final acceptance of the products, whichever is later. Payment terms offering a discount will not be considered in the price evaluation of your offer.
____ Contractor may be paid immediately upon invoice approval, if enrollment is made to the
Single Use Account (SUA) Program, administered by the City’s servicing bank (“Bank”). By checking this box, the vendor accepts transaction costs charged by their merchant bank and agrees not to transfer to the City those extra charges. The City will not pay an increase in our services for the SUA charges; if an audit uncovers an upcharge for the SUA charges the vendor will owe the City all costs. The vendor may opt-out of the SUA program once, but then may not rejoin during the same contract term. For more information about the SUA program or to enroll, send email to mailbox.sua@phoenix.gov.
