About this file

This is a Subcontractor Summary Worksheet form issued by the City of St. Petersburg, Florida, for use in bidding on municipal construction projects. The document serves as a mandatory submission requirement with all bids and establishes the framework for subcontractor management throughout the contract term. Prime contractors must complete the worksheet to identify all intended subcontractors, including their legal names, primary business addresses, contact information for project supervisors, trade classifications, and subcontract amounts. The form permits first and second-tier subcontractors only, unless otherwise specified in contract documents, and establishes specific procedures for subcontractor changes post-award, limiting substitutions to circumstances where subcontractors fail to execute contracts, lack appropriate licensing, become insolvent, prove ineligible, fail to perform, deliver substantially unsatisfactory work, or substantially delay project progress. Requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management office.

The worksheet requires bidders to identify the percentage of contract work permitted for subcontracting and includes provisions for Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractor participation. SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates SBE requirements are satisfied through alternative means. Restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects. The form includes a legend identifying seven specific reason codes for authorized subcontractor deletions, requiring documentation and City approver authorization with dates for all additions or deletions to the subcontractor list.
