About this file

This file is a Subcontractor Summary Worksheet instruction document from the City of St. Petersburg, Florida, designed to accompany bids for the Skate Park Obstacle Demolition/Replacement project at Campbell Park in St. Petersburg. The worksheet governs the use and management of subcontractors on the project, permitting up to 80% of the contract work to be subcontracted. Bidders must complete and submit the worksheet with their bid proposals, listing all intended subcontractors with their legal names, business addresses, contact information, trade classifications, and subcontract amounts. Only first and second tier subcontractors are permitted unless otherwise specified in contract documents.

The instructions establish strict restrictions on subcontractor changes after contract award and the pre-construction meeting, allowing substitutions only under specific circumstances including failure to execute contracts, inadequate licensing, bankruptcy, ineligibility, poor performance, or substantial work delays. Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet percentage requirements must be replaced with similarly certified firms if substituted. The worksheet requires identification of the prime contractor, the individual completing the form, and approval tracking through the City's Contracts Compliance Manager. Restrictions on subcontracting percentages and multi-tier subcontractors do not apply to FDOT LAP or federally funded projects.
