About this file

This is a Subcontractor Summary Worksheet template from the City of St. Petersburg, Florida, designed to document and manage subcontractor participation for City projects. Prime contractors are required to complete and submit this worksheet with their bids, detailing all intended subcontractors and their assigned work scopes. The worksheet captures essential subcontractor information including company name, business address, project supervision contact details, trade classification, subcontract amount, and tier level (first or second tier only, unless otherwise specified in contract documents). The City maintains a mechanism for tracking subcontractor changes and deletions through designated reason codes, with all modification requests directed to the Procurement & Supply Management office at Stephanie.Swinson@stpete.org.

Subcontractor substitutions are generally prohibited after award and pre-construction meeting, with limited exceptions including subcontractor insolvency, licensing deficiencies, performance failures, or substantial project delays. When Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors are utilized to meet contractual SBE percentage requirements, any replacement must also be an SBE contractor unless the prime demonstrates alternative SBE compliance methods. The worksheet requires prime contractors to indicate whether no subcontractors will be used and provides a calculation section for determining allowable subcontracting percentages relative to total contract amounts. Restrictions on subcontracting percentages and multiple-tier subcontractor usage do not apply to Florida Department of Transportation Local Agency Program (FDOT LAP) or federally funded projects.
