About this file
This is a Subcontractor Summary Worksheet document issued by the City of St. Petersburg, Florida, that serves as an instructional and operational template for managing subcontractor information on municipal construction and service projects. The worksheet requires prime contractors to disclose all intended subcontractors, their business details, trade classifications, and subcontract amounts prior to bid submission. The document establishes that subcontractors may not be changed or substituted after a prime contractor is awarded the project except under specific circumstances, including failure to execute contracts, lack of appropriate licensing, bankruptcy, ineligibility, unsatisfactory performance, or substantial delays to project progress. The worksheet accommodates tracking of both first and second-tier subcontractors and includes provisions for Small Business Enterprise (SBE) and Minority/Women Business Enterprise (MWBE) designations.
The worksheet provides a structured format for documenting subcontractor information, including complete legal names, primary business addresses, contact information for project supervision personnel, and specific trade classifications being subcontracted. Prime contractors must enter the allowable percentage of contract work that may be subcontracted, with the total subcontract amount calculated against the overall contract amount to ensure compliance with specified limitations. Notably, restrictions on subcontracting percentage and use of multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects. Any requests to modify the subcontractor list after award must be submitted in writing to Procurement & Supply Management with documented justification using prescribed reason codes. SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors if substitution becomes necessary, unless the prime contractor demonstrates that SBE requirements are being satisfied through alternative means.
