About this file

This is a Subcontractor Summary worksheet instruction document from the City of St. Petersburg, Florida, applicable to a shelter project with an 80% subcontracting allowance. The worksheet serves as a mandatory submission requirement with each bid and requires bidders to identify their company as the prime contractor, list all intended subcontractors with complete legal names and business addresses, provide contact information for project supervision personnel, specify trade classifications being subcontracted, and document subcontract amounts. The document establishes that subcontractors cannot be changed or substituted after contract award and pre-construction meeting except under specific circumstances including failure to execute contracts at specified prices, inappropriate licensing, bankruptcy or insolvency, ineligibility, failure or refusal to perform work, substantially unsatisfactory work quality, or substantial delays to project progress.

SBE subcontractors used to meet percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates SBE requirements are being met through alternative means. The worksheet includes fields for tracking additions and deletions of subcontractors with corresponding reason codes and approval signatures from the City's procurement department. Requests for subcontractor changes must be submitted in writing to the Contracts Compliance Manager. The document notes that restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects. Only first and second tier subcontractors are permitted unless otherwise specified in contract documents.
