About this file

This is a Subcontractor Summary Worksheet template from the City of St. Petersburg used for construction and procurement projects. The document serves as a standardized form that prime contractors must complete and submit with their bids to identify all intended subcontractors and provide detailed information about subcontracting arrangements for projects at the City of St. Petersburg, including work at the Albert Whitted Airport.

The worksheet requires prime contractors to list each subcontractor's legal name, business address, primary contact information, tier classification (first or second tier only unless otherwise specified), trade classifications, and subcontract amounts. The form tracks whether subcontractors are Small Business Enterprises (SBE) or Minority-Women Business Enterprises (MWBE) and includes provisions for adding or deleting subcontractors post-award with documented reasons. Subcontractors may only be changed after contract award for seven specific reasons: failure to execute a written contract at the specified price, lack of appropriate licensure, bankruptcy or insolvency, ineligibility, failure to perform, substantially unsatisfactory work performance, or substantial delay or disruption of project progress. All requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management Contracts Compliance Manager. The worksheet calculates total subcontract amounts against the allowable percentage of work permitted to be subcontracted as specified in the contract documents, with restrictions on subcontracting percentages and multiple-tier subcontractors not applying to FDOT LAP or federally funded projects.
