About this file
This is a Subcontractor Summary Spreadsheet template from the City of St. Petersburg establishing requirements for subcontractor disclosure and management on municipal construction projects. The worksheet must be completed and submitted with all bids and requires prime contractors to identify all intended subcontractors, their business addresses, contact information, trade classifications, and subcontract amounts prior to award. The document establishes that subcontractors may not be changed or substituted after a prime contractor is awarded and a pre-construction meeting is held, except under specific circumstances including failure to execute a contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform the subcontract, substantially unsatisfactory work quality, or substantial delays or disruptions to project progress. Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates SBE requirements are met through alternative means.
The spreadsheet accommodates documentation of first and second tier subcontractors only, unless otherwise noted in contract documents, and includes fields for tracking additions and deletions with corresponding reason codes and city approver authorization. Requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org. The template provides a total line for calculating subcontract amounts against the contract amount to determine the allowable percentage of work that may be subcontracted. This worksheet applies to the City of St. Petersburg's procurement for a Manhole Inspection Unit for the Water Resources Department, which seeks a camera system to inspect the collection system's 19,406 manholes for structural assessment and regulatory compliance purposes.
