About this file

This is a Subcontractor Summary worksheet issued by the City of St. Petersburg for a sheltered contract opportunity related to a Manhole Inspection Unit procurement. The document establishes the framework for subcontractor participation on the project, permitting up to 80% of contract work to be subcontracted. Prime contractors must complete and submit this worksheet with their bid, providing detailed information on all intended subcontractors including their legal names, business addresses, contact information, trade classifications, and subcontract amounts. The worksheet specifies that only first and second tier subcontractors are permitted unless otherwise noted in the contract documents, and it identifies seven conditions under which subcontractor substitutions may be authorized after contract award, including failure to execute agreements, inadequate licensing, bankruptcy, ineligibility, non-performance, unsatisfactory work quality, or substantial project delays.

The document emphasizes that Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substituted, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. Requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management department, specifically to the Contracts Compliance Manager. The worksheet includes fields for tracking added and deleted subcontractors with reason codes corresponding to the authorized substitution conditions. These instructions represent the City of St. Petersburg's standard subcontracting protocol and are mandatory components of the bidding process for this sheltered contract opportunity.
