About this file

This is a Subcontractor Summary Worksheet instruction document from the City of St. Petersburg, Florida, applicable to the procurement for an electric wheelchair lift installation at the President Barack Obama Main Library auditorium stage. The worksheet serves as a mandatory submission requirement with all bids and establishes the framework for identifying subcontractors intended for the project. Bidders must enter their company name as the prime contractor, provide the name and date of the completing employee, and list all intended subcontractors with complete legal names, primary business addresses, contact information for project supervision personnel, tier classification (first or second tier only unless otherwise noted), trade classifications, and subcontract amounts. The solicitation released on March 10, 2026, with a question deadline of March 12, 2026, and response deadline of March 18, 2026, at 3:00 p.m., allows prime contractors to subcontract up to 80 percent of the contract work.

Subcontractors identified at bid time cannot be changed or substituted after contract award and the pre-construction meeting except under seven specific circumstances: failure or refusal to execute a written contract at the specified price, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform the work, failure or refusal to perform the subcontract, substantially unsatisfactory work performance not in accordance with plans and specifications, or substantial delays or disruptions to project progress. Requests for subcontractor changes must be submitted in writing to Procurement & Supply Management, Attention: Contracts Compliance Manager at Stephanie.Swinson@stpete.org. If Small Business Enterprise (SBE) subcontractors are used to meet SBE percentage requirements, replacement subcontractors must also be SBEs unless the prime contractor demonstrates SBE requirements are being met through alternative means. The document applies to standard city projects; restrictions on subcontracting percentage and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects.
