About this file

This is a Subcontractor Summary worksheet document issued by the City of St. Petersburg, Florida, for use in bidding on the wheelchair lift installation project for the President Barack Obama Main Library auditorium stage. The worksheet is a mandatory submission requirement that must accompany all bids and serves to identify all intended subcontractors, their qualifications, and the scope of work they will perform on the project. Prime contractors are permitted to subcontract up to 80 percent of the contract work. The document requires bidders to provide detailed information for each subcontractor, including the subcontractor's legal name, primary business address, contact information and designated project supervisor, trade classifications, subcontract amount, and tier level (first or second tier only, unless otherwise specified in contract documents). Subcontractors cannot be changed or substituted after contract award and preconstruction meeting, except under seven specific circumstances: failure to execute a written contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform the work, failure to perform the subcontract, substantially unsatisfactory work performance, or substantial delays or disruption to project progress.

Any requests to modify the subcontractor list following award must be submitted in writing to the City of St. Petersburg Procurement & Supply Management department, attention Contracts Compliance Manager at Stephanie.Swinson@stpete.org, along with the applicable reason code. For projects with Small Business Enterprise (SBE) or Minority/Women-Owned Business Enterprise (MWBE) contractor percentage requirements, any SBE subcontractor used to meet those requirements must be replaced with another SBE subcontractor if substitution becomes necessary, unless the prime contractor can demonstrate that SBE requirements are being met through alternative means. Restrictions on subcontracting percentages and multi-tier subcontractor use do not apply to FDOT LAP or federally funded projects.
