About this file
This file is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg, Florida, designed to accompany competitive bids for municipal projects. The worksheet requires prime contractors to disclose all intended subcontractors, including their names, business addresses, contact information, trade classifications, and subcontract amounts. Bidders must complete this form and submit it with their bid proposals, or provide a written statement if no subcontractors will be used. The worksheet accommodates tracking of first and second-tier subcontractors only, unless otherwise specified in contract documents, and includes provisions for documenting additions or deletions of subcontractors throughout the project lifecycle.
Subcontractor substitutions are prohibited after contract award and the pre-construction meeting, except in specific circumstances including contractor failure or refusal to execute the subcontract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform the subcontract, substantially unsatisfactory work performance, or substantial project delays. Any requests for subcontractor changes must be submitted in writing to the Procurement & Supply Management department, Attention: Contracts Compliance Manager at Stephanie.Swinson@stpete.org. Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors if substitution becomes necessary, unless the prime contractor demonstrates that SBE requirements are being met through alternative means. Restrictions on subcontracting percentages do not apply to FDOT LAP or federally funded projects.
