About this file

This is a Subcontractor Summary worksheet form issued by the City of St. Petersburg, Florida, providing instructions and a template for prime contractors to disclose and track subcontractor information on contracted projects. The form requires prime contractors to identify all intended subcontractors prior to bid submission, including their legal names, business addresses, contact information, and the specific trade classifications for which they will perform work. The worksheet establishes that up to 80 percent of the contract work may be subcontracted for shelter projects. Subcontractors may only be changed or substituted after project award under specific circumstances, including failure to execute a written contract, lack of proper licensing, bankruptcy, ineligibility, performance failures, substantial deficiencies in workmanship, or significant project delays. Any requests for subcontractor substitutions must be submitted in writing to the City's Contracts Compliance Manager.

The form includes specific provisions for Small Business Enterprise (SBE) and Minority/Women Business Enterprise (MWBE) subcontractors, requiring that if an SBE subcontractor used to meet the SBE percentage requirement is replaced, it must be replaced with another SBE subcontractor unless the prime contractor demonstrates that SBE requirements are being met through alternative means. The worksheet designates space for tracking subcontractor additions and deletions with corresponding reason codes and city approval documentation. The form notes that restrictions on subcontracting percentage and multiple-tier subcontractors do not apply to Federal Department of Transportation (FDOT) Local Agency Program (LAP) projects or other federally funded projects. The worksheet was last revised in January 2020.
