About this file

This is a Subcontractor Summary Spreadsheet template issued by the City of St. Petersburg, Florida, designed to accompany bid submissions for municipal projects. The spreadsheet serves as a mandatory worksheet that prime contractors must complete and submit with their bids, documenting all intended subcontractors and their respective scopes of work. The form requires contractors to identify themselves as the prime contractor, list detailed information for each subcontractor including legal name, business address, project supervision contact information, trade classifications, and subcontract amounts, and specify whether subcontractors are Small Business Enterprises (SBE) or Minority/Women-Owned Business Enterprises (MWBE). The template provides for designation of first or second tier subcontractors only, unless otherwise specified in contract documents, and includes fields for tracking additions and deletions to the subcontractor list with corresponding reason codes.

Subcontractor changes after contract award and the pre-construction meeting are prohibited except under specific circumstances, including subcontractor failure to execute written contracts, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, performance failures, unsatisfactory work quality, or substantial project delays. Any SBE subcontractors utilized to meet SBE percentage requirements must be replaced with other SBE subcontractors if substitution becomes necessary. Requests for subcontractor changes must be submitted in writing to the City's Procurement & Supply Management Contracts Compliance Manager. The instructions clarify that restrictions on subcontracting percentages and multiple-tier subcontractor use do not apply to FDOT LAP or federally funded projects. If no subcontractors will be used, contractors must provide written notification on the worksheet indicating this determination.
