About this file

This is a Subcontractor Summary worksheet issued by the City of St. Petersburg, Florida, for use in the Community Engagement Consulting Services contract opportunity. The worksheet must be completed and submitted with all bids. Bidders are required to identify their company as the prime contractor, list all intended subcontractors with complete legal names and primary business addresses, provide contact information for the subcontractor's project supervisor, specify the trade classification and tier level (first or second tier only) for each subcontractor, and indicate the subcontract amount. The form allows for tracking additions and deletions of subcontractors throughout the contract period, with deletions permitted only under specific circumstances including failure to execute a contract, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure or refusal to perform, substantially unsatisfactory work performance, or substantial delays to project progress. Any changes to the subcontractor list must be submitted in writing to the Procurement & Supply Management office, specifically to the Contracts Compliance Manager at Stephanie.Swinson@stpete.org.

For this shelter project, up to 80 percent of contract work may be subcontracted. Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors used to meet SBE percentage requirements must be replaced with another SBE subcontractor if substituted, unless the prime contractor demonstrates SBE requirements are being met through alternative means. The restrictions on subcontracting percentages and multi-tier subcontractor use do not apply to Federal-Aid Program (FDOT LAP) or federally funded projects. The worksheet includes a legend identifying seven specific reasons for subcontractor deletion and requires the city approver's signature and date for any modifications made after contract award.
