About this file
This is a Subcontractor Summary Worksheet template issued by the City of St. Petersburg for use on municipal construction and service projects. The worksheet serves as a mandatory submission requirement for all bidders and must accompany project bids. The form requires prime contractors to identify all intended subcontractors, including their legal names, business addresses, contact information, and the specific trade classifications for which they will be responsible. Bidders must specify whether subcontractors will be used and provide the percentage of contract work to be subcontracted. The worksheet accommodates tracking of Small Business Enterprise (SBE) or Minority/Women Business Enterprise (MWBE) subcontractors and distinguishes between first and second-tier subcontractors. Once a prime contractor is awarded a project and the pre-construction meeting is held, subcontractor changes are restricted and require written approval from the Procurement & Supply Management's Contracts Compliance Manager.
Subcontractor substitutions may only occur for specific documented reasons, including failure to execute contracts, lack of appropriate licensing, bankruptcy or insolvency, ineligibility to perform work, failure to perform, substantially unsatisfactory work quality as determined by the engineer, or substantial delays or disruptions to project progress. SBE subcontractors used to meet SBE percentage requirements must be replaced with other SBE subcontractors unless the prime contractor demonstrates SBE requirements are being met through alternative means. Restrictions on subcontracting percentages and multiple-tier subcontractors do not apply to FDOT LAP or federally funded projects. The worksheet includes fields for tracking additions and deletions of subcontractors with associated reason codes and city approver authorization and dates.
