About this file
This is a Subcontractor Summary Worksheet issued by the City of St. Petersburg for a sheltered project related to the preservation of historical engineering records. The project involves converting historical engineering records stored on microfiche, aperture cards, and microfilm into searchable PDF files. The worksheet must be completed and submitted with all bids and establishes that up to 80 percent of the contract work may be subcontracted. Prime contractors are required to provide detailed information for each intended subcontractor, including the subcontractor's legal name, primary business address, project supervision contact information, trade classifications, and subcontract amount. The document specifies that subcontractors cannot be changed or substituted after contract award and the pre-construction meeting without documented justification, such as failure to execute a written contract, lack of appropriate licensing, bankruptcy, ineligibility, poor performance, or substantial project delays.
The worksheet indicates specific conditions for subcontractor replacement, particularly requiring that any Small Business Enterprise (SBE) subcontractors used to meet SBE percentage requirements must be replaced with another SBE subcontractor unless the prime contractor demonstrates alternative compliance with SBE requirements. Changes to the subcontractor list after award must be submitted in writing to the City's Procurement & Supply Management office. The document notes that restrictions on subcontracting percentages and multiple-tier subcontractor limitations do not apply to FDOT LAP or federally funded projects. Only first and second tier subcontractors are permitted unless otherwise noted in the contract documents. All subcontractor information is subject to city approval and must include dates of approval or deletion with corresponding reason codes.
