Vendors must provide comprehensive insurance coverage including $1 million in Commercial General Liability, Commercial Automobile Liability, and Professional Liability insurance, along with Workers' Compensation coverage. The City encourages participation from Minority Business Enterprises (MBE), Small Business Enterprises (SBE), Woman-Owned Business Enterprises (WBE), and Disadvantaged Business Enterprises (DBE), offering additional evaluation points for these certifications. Vendors must comply with E-Verify requirements, public records laws, and mandatory background checks for employees accessing City facilities. Projects funded by FDOT are contingent upon legislative appropriation and subject to change, with all non-City-funded projects required to conform to the applicable funding agency's requirements.

Appendix C - Statement of Qualifications Requirements Miscellaneous Professional Services Construction and Engineering Inspection

The Request for Qualifications shall include the following information in the order listed. Upload all responses as one PDF labeled “Statement of Qualifications Requirements”. Page size shall be limited to 8 ½ x 11.

The Bid Proposal shall be limited to 75 pages. Proposals in excess of 75 pages may be deemed non-responsive by the Procurement and Supply Management Director. A cover page or section divider pages are not included in the total page count.

While the use of SF330 Forms is not required, the Proposal must include all information as outlined in the SF330.

B. Transmittal Letter

Offerors must submit a transmittal letter on the firm’s letterhead. It must be signed by a representative of the offeror authorized to sign such material and to commit the offeror to the obligations contained in the SOQ. The transmittal letter must include the name, address, phone number and e-mail address for the offeror’s contact, and must specify who would be the firm’s signatory to any contract documents executed with the City. The transmittal letter may include other information deemed relevant by the offeror.

C. Table of Contents/PDF Bookmarks

Include a Table of Contents and Bookmark each section of the PDF.

D. Project Team (SF 330, Sections A – E)

In this section, the offeror must use SF 330 Part I, A – E to describe the composition and organization of the team.

Identify any other firms (such as sub-contractors and sub-consultants) included on the project team along with the offeror.

The organizational chart should indicate the reporting relationships and responsibilities of the offeror, including any subconsultants and subcontractors.

Key personnel shall include the offeror's project manager, at least one (1) substitute project manager, and other individuals with specific skills relevant to the anticipated work. The proposed project manager and the proposed substitute shall have a minimum of ten (10) years of applicable experience and shall remain with the project throughout the term of the City's standard agreement. The project manager shall not be replaced or substituted without the prior approval of the City. The City may request an interview with the short-listed firm's proposed project managers. The key project manager or their approved substitute(s) shall be available for all meetings. Experience included on the resume(s) must be limited to projects completed within the past ten (10) years.

E. Experience and Reference Projects (SF 330, Sections F – G)

In this section, the offeror must use SF 330 Part I, F – G to describe the performance history and experience of the project team on similar projects.

The information should demonstrate the firm’s ability to work cooperatively with multiple clients and any third-party. Submit a combination of five (5) reference projects and continuing services contracts completed in the past ten (10) years that demonstrate relevant experience. For continuing services contract references, include a list with a single sentence description of the project(s) completed under the continuing services contract.

FDOT Projects and LAP Projects The experience and reference projects should, at a minimum address the following technical requirements:

The offeror shall submit descriptions of reference projects to demonstrate relevant experience. Each project description shall contain at least the following information: (a) name of owner, (b) owner reference and contact information, (c) role of offeror, (d) contract value, (e) year started and year completed, (f) description of the project showing relevance to this project with emphasis on the offerors experience with EEO Compliance and Reporting, (g) names of firms and key personnel that participated in similar project(s) and are included in this offer, along with a clear description of the roles and responsibilities of each, and (h) provide a summary table to cross-reference the project team (firms and key personnel) with participation in the reference projects.

F. Additional Information (SF 330, Section H)

1. Firm Specific Qualifications The offeror should identify the key strengths of the firm. Highlight capabilities and areas of specialization provided by the firm.

2. Project Approach The offeror should identify the process utilized on other continuing services contracts and the process envisioned for the services identified in this RFQ to ensure task authorizations meet expectations. This should include the process envisioned for interfacing with the City through the project manager. If sub-consultants are part of the team, describe the specific scope of work for each firm’s services and specifically in which types of projects the subconsultants will participate. Describe the firms approach to the management of subconsultants and subcontractors.

3. Safety Provide a discussion on the firm’s approach to safety.

4. CADD and GIS capabilities Provide a discussion on the firm’s CADD and GIS capabilities. The City currently uses AutoCAD Civil 3D 2024, ESRI ArcMap 10.8 & ESRI ArcPro Version 3.0. All submitted electronic drawings will need to be compatible with this version of CADD.

5. General Qualifications (SF 330, Part II) Provide one (1) updated SF 330 Part II, for the offeror's office(s) which will provide services identified in this RFQ. Provide the same information for each subconsultant.

6. References Provide a list of three (3) client contact persons for whom the offeror has recently provided services similar in nature to the services identified in this RFQ.

7. City Experience Provide a list projects, including dates and contract amounts for previous and current work performed by offeror for the City of St. Petersburg.

8. Contractual Failures Describe any alleged, significant prior or ongoing contract failures, licensing or permit violations, claims on bonds, any civil or criminal litigation or investigation pending (including wage theft complaints filed) which involves the firm or in which the firm has been judged guilty or liable.

9. Supplier Diversity / Business Outreach Plan Whether or not the offeror or any of its subconsultants or subcontractors is a certified Minority Business Enterprise (MBE) as defined by the Florida Small and Minority Business Assistance Act, a Woman Owned Business Enterprise (WBE) a Disadvantaged Business Enterprise (DBE), and/or a Small Business Enterprise (SBE). If the offeror and/or its subconsultants or subcontractors are a MBE, WBE, DBE, or SBE, provide the name of the firm and the certifying agency in a matrix. This may include a Business Outreach Plan.

10. Licenses Provide copies of offeror's and individual’s current applicable Florida License.
