About this file

State Form 96 is a standardized bid form prescribed by the Indiana State Board of Accounts for public works projects and is used by governmental units throughout Indiana, including the City of Bloomington's Board of Public Works. This form serves as the official contractor's bid submission document for public works projects and requires bidders to provide comprehensive project information, organizational experience, equipment and subcontracting plans, financial statements, and sworn affidavits. For the Parking Lot Rehabilitation project involving three parking lots in Bloomington (located at 4th & Dunn, 4th & Washington, and 6th & Lincoln), bidders must submit a completed State Form 96 along with supporting documentation including a Non-Collusion Affidavit, E-Verify Affidavit, and contract compliance certification. The sealed bid submission deadline is December 15, 2025 at 12:00 p.m. through the OpenGov e-Procurement Portal, with a public bid opening scheduled for the same date. The project encompasses rehabilitation of approximately 63,650 square feet of parking lot area and includes milling, surface material application, ADA compliance modifications, meter pole installation and removal, bumper block replacement, signage straightening, re-striping, curb capping, and debris removal. Work is scheduled to commence on June 1, 2026, with a required completion timeline of 45 calendar days, weather permitting.

State Form 96 mandates that for projects exceeding $150,000, bidders must complete Part II of the form, submitting detailed responses regarding past public works project experience, current construction projects in progress, organizational equipment and resources, subcontracting plans, and a mandatory financial statement demonstrating capability to complete the project. The form requires bidders to certify compliance with non-collusion provisions and affirm under penalty of perjury that all submitted information is true and correct. Additionally, the City of Bloomington requires strict compliance with its Living Wage Ordinance, establishing a 2026 wage rate of $16.66 per hour (with up to 15 percent potentially provided through health insurance), compliance with federal equal employment opportunity laws and the Bloomington Human Rights Ordinance, and submission of a written employee drug testing program plan for contracts exceeding $150,000. Performance and payment bonds equal to 100 percent of the contract price are mandatory for contracts of $200,000 or more, with a one-year maintenance period following final acceptance. Contractors must also demonstrate compliance with Indiana Department of Transportation Specifications and environmental regulations, including erosion control requirements.
