About this file

This is a Contractor's Bid Form (State Form 96) prescribed by the Indiana State Board of Accounts for use on public works projects undertaken by governmental units in Indiana, including the City of Bloomington's Board of Public Works. The form is designed for contractors to submit sealed bids for public works projects in accordance with established plans and specifications. Bidders are required to provide comprehensive project information including their firm's name, address, contact information, and designated agent. The form accommodates both standard and alternative bids, with contractors obligated to furnish labor and materials necessary to complete the project and provide bonds or certified checks as specified in the notice of letting. For projects valued at $150,000 or more, bidders must complete additional sections documenting their experience with prior public works projects, current construction activities, project completion history, equipment and subcontractor availability, and detailed plans for executing the proposed work.

The form requires mandatory submission of a bidder's financial statement to enable the governmental unit to assess the contractor's capability to complete the project. All bidders must execute a non-collusion affidavit confirming they have not engaged in price-fixing or collusive agreements with other bidders, and must provide an oath or affirmation under penalty of perjury that all submitted information is true and correct. The form mandates compliance with federal steel product requirements under Indiana Code 5-16-8-2, requiring contractors and subcontractors to use U.S.-made steel products on the project. Additional requirements include prohibitions against employment discrimination based on race, religion, color, sex, national origin, or ancestry, with breach constituting material contract violation. Contractors must provide notarized acknowledgment of their bid submission, with the form serving as a comprehensive legal and administrative document for the competitive bidding process.
