About this file
This file contains the required forms and certifications for a Construction Manager at Risk (CMAR) procurement issued by the Nevada Department of Transportation (TTD) for the SR 28 Central Corridor Trail project extending from Sand Harbor to Thunderbird Cove. The RFP is a single-step procurement process in which qualified general contractors submit proposals addressing pre-construction CMAR services and potential construction services, with selection based on qualifications evaluation and subsequent interview scoring. The project scope encompasses safety improvements, a shared-use path, and environmental sustainability initiatives along State Route 28. TTD expects the Construction Manager and team members to be general contractors rather than engineering firms, and the procurement will culminate in negotiations preceding execution of the Pre-Construction Services Agreement and potential Construction Contract contingent upon acceptance of the Construction Manager's Construction GMP or fixed price bids.
The submission package requires proposers to complete multiple compliance and disclosure forms including Buy America and Build America certification, breach of contract and debarment certifications, organizational conflict of interest disclosure, lobbying activity disclosures, non-collusion affidavits, and industrial safety records for all team members performing installation or construction work. Proposers must provide Key Personnel information and demonstrate compliance with Federal Highway Administration requirements, Nevada Revised Statutes provisions, and applicable federal regulations including suspension and debarment certifications. The safety record documentation requires submission of OSHA Form 300A data, incidence rates, and Insurance Experience Modification ratings for both nationwide and Nevada-specific operations for years 2022 through 2025, along with disclosure of any significant incidents or OSHA citations.
