PERFORMANCE REQUIREMENTS SUMMARY
1. ontract requirements listed throughout the Statement of Work summarize specific firm fixed-price tasks that are to be performed under this contract. The Contractor’s work is based on recurring work and monthly work tickets as specified in the Historical Data, Attachment C8.
2. During each month, the Government will compare Contractor performance to the contract requirements through inspections of work performed. The Government will evaluate each required service based on the following inspection methods:
a. COR Inspections (Monthly Inspection/QDR’s Issued)
b. Work In Progress - Indefinite Quantity Work (IDQ)
3. Performance of a required service is considered Satisfactory when the number of defective units (unsatisfactory outputs) found by the Government during contract surveillance does not have to be re-performed. When the number of defective units discovered by the COR causes re-work by the Contractor, the Contractor’s overall performance for that type of service is considered unsatisfactory. The COR will issue the Contractor a Quality Deficiency Report (QDR) for re-work. The Contractor shall be required to respond in writing to a QDR by entering the information into the SPAN FM system upon completion. The QDR will require the Contractor to explain, in writing, why performance was unacceptable, how performance will be returned to satisfactory levels, and how recurrence of the problem will be prevented in the future.
4. The Contractor shall re-perform, without additional cost to the Government, ALL work found to be unsatisfactory. The COR will determine the amount of time the Contractor will be given to re-perform the work on a case by case basis. Re-performance will not improve the overall rating of the service in question and the QDR counts towards the Government’s Monthly Inspection Service Count.
5. For unsatisfactory services performed that exceed a 5% limit to the estimated quantities listed under Historical
Data, Attachment C9, throughout any given month, the Government reserves the right to deduct for ALL services re-performed and/or not provided. Work tickets are averaged out at $100.00 per ticket. If the Contractor’s QDR count is within the Government’s limit of 5%, the Government will not take a deduction for the month. If the Contractor’s QDR count exceeds the Government’s 5% limit, the Government has the option to deduct for the full QDR count for the month at $100.00 per work ticket.
EXAMPLE: Average work ticket count of 89 per month. If the Contractor’s Quality Deficiencies count does not exceed the given 5%, there will be no deduction for the month. If the Contractor exceeds the 5% given by the Government, the full percentage will be deducted as follows:
Monthly Work Ticket Count: 89 $100.00 QDR’s Issued for the Month: 5/89 .05% No Deductions Taken
Monthly Work Ticket Count: 89 $100.00
QDR’s Issued for the Month: 10/89 .11% Deductions for 10 work tickets at $100.00 per work ticket or $1000.00 would be deducted from the Contractor’s Monthly Invoice.
6. Monthly payment to the Contractor will be reduced for unsatisfactory performance. If performance of required service is unsatisfactory, and the unsatisfactory performance is clearly the fault of the Contractor, an average amount of $100.00 per work ticket will be deducted from the Contractor’s monthly payment once the limit of 5% has been exceeded.
7. Indefinite Quantity (IDQ) work will be monitored and inspected as work is ongoing and as outlined in Clause B.38. The Contractor will be deducted 1% for each day the delivery order is not completed by the delivery order due date. The maximum deduction in price will not exceed 15% of the total delivery order price.
