STATEMENT OF WORK
Statement of Work
FY12 Avaya Maintenance
BACKGROUND
The Information Technology Division at the Joint Forces Staff College (JFSC) is responsible for providing innovative solutions by leveraging people, processes, and technology to ensure a secure and reliable information exchange environment that exceeds customer expectations.
JFSC's technical environment consists of voice, video, and data computing and networking facilities.
OBJECTIVE
The objective of this effort is to obtain telephone switch and voicemail system service maintenance for the Avaya Definity on the telephone switch, Audix Voicemail system, and subassemblies installed at the
JFSC.
SCOPE OF WORK
Due to the unpredictability of service outages, JFSC requires contractor's services on an as-needed basis to provide business-day corrective maintenance and support. Support entails full maintenance coverage of the switch, 8 hours a day, Monday to Friday. Contractor must be Avaya certified to provide switch maintenance support on Avaya equipment and infrastructure;
specifically the contractor must have full system access to maintenances service permissions
(MSPs) of the Avaya system.
PLACE OF PERFORMANCE
All work shall be performed either on-site at JFSC or via remotely with approved access to the switch.
Joint Forces Staff College
7800 Hampton Blvd
Norfolk, VA 23511
PERIOD OF PERFORMANCE: The period of performance of the maintenance requirement is one (1) year from issue date of the contract plus two option years.
SAFETY AND SECURITY:
The requirements for access to the Naval Support Activity Base (NSA) upon which JFSC resides requires contractors (to include delivery companies) to be registered with RapidGATE. Should a vendor chose not to register with the RapidGATE system; the following process must be followed to gain access to NSA:
Access requests for unescorted visitors must be submitted to NSA no less than 5 days in advance of the scheduled visit. A list of visitors containing personal information, to include name, DOB and social security number, will be submitted to the NSA Hampton Roads security department at least 5 days prior to the scheduled visit in order for NRMA N3 personnel to cross check the
National Crime Information Center database, the Terrorist Screening Database and the Sexual
Offender and Registry Database. On the day of the event, visitors must go to the Pass and ID office on Hampton Blvd to be issued a 1-day pass. Visitors are required to renew passes at the
Pass and ID Office on Hampton BLVD daily. To maintain privacy, JFSC does not receive notification as to whether individuals are cleared or if vendor personnel are denied access to the
NSA Complex. Once a visitor is cleared, he/she will have access for at least 3 months.
The contractor shall be responsible for ensuring contractor personnel assigned to work on this contract:
- Satisfy JFSC access security requirements by reporting to the Quarter Deck at 757-443-6076 upon arrival to the College and providing job information to the individual at the desk. Additionally, the technician will be required to sign in on a Visitor Control log and be escorted in the switch area when working on the equipment.
- Adhere to general safety practices when working on energized equipment.
NON-PERSONAL SERVICES STATEMENT
Contractor employees performing services under this order will be controlled, directed, and supervised at all times by management personnel of the contractor. Contractor management will ensure that employees properly comply with the performance work standards, outlined in the statement of work. Contractor employees will perform their duties independent of, and without the supervision of, any Government official or other Defense Contractor. The tasks, duties, and responsibilities set forth in this effort may not be interpreted or implemented in any manner that results in any contractor or sub-contractor employee creating or modifying Federal policy, obligating the appropriated funds of the United State Government, overseeing the work of
Federal employees, providing direct personal services to any Federal employee, or otherwise violating the prohibitions set forth in Parts 7.4 and 37.1 of the Federal Acquisition Regulation

Should the ―Option to Extend Services‖ clause be exercised, pricing will follow past practices and be set at the then current rates being utilized in the contract, prorated to the Extended Services performance periods being utilized. For example, should this clause be exercised for a two month performance period to follow the option year one performance period of the contract, then the pricing would be set for two months at option year one pricing, barring the operation of other law."
