1. Per subject solicitation requirements, the facilities and transporters need to be on the QFL and QTL. I would appreciate it if you could let me know what the timing is to become qualified and on the list as a facility and as a transporter?
a. We typically respond to requests for additions to the QFL and QTL within five business days. If additional documentation is needed or there are additional questions we need to ask the contractor, approval can take as long as 3 or 4 weeks, depending on the issue and/or how quickly the contractor responds to our requests for additional information. For a transporter or facility to be placed on the QTL/QFL, respectively, the Primary Contractor must submit one application packet per transporter or facility with the following documents to: TSDFandTransporterInquiries@dla.mil
A complete facility application packet should include the following elements:
1) Completed DLA form 2507 (Nov 2023), attached. Page one and two of this form needs to be completed with all sections filled or NA utilized. Page four is for DLA use only.
2) ACORD Certificate of Liability Insurance showing the site address. The ACORD requires commercial general liability and some form of pollution insurance that does not expire within 30 days of review by DLA. Some facilities also require closure insurance, if so, provide updated banking or regulatory documents showing the site's address has coverage with an expiration date that has not expired.
3) Third party inspection no older than 18 months. This documentation shall be from the third party that performed the inspection, and it shall include what was inspected at the site's address, the findings, and any corrective actions.
4) Regulator permit(s) or business license(s) for the authorized activities at the site showing the site address with an expiration date that has not expired. *** Identify in your email response whether or not the site is authorized to receive manifested regulated waste codes. If the facility is not a RCRA-permitted facility and has licensing or an exemption to receive manifested regulated waste codes, that documentation authorizing this authority must be provided.
A complete transporter application packet should include the following elements:
1) Completed DLA form 2503 (MAR 2022), attached. This form needs to be completed with all sections filled or NA utilized. The bottom section of page one is for DLA use only.
2) ACORD Certificate of Liability Insurance showing the company address. The ACORD requires the following insurance that does not expire within 30 days of review by DLA: auto insurance, commercial general liability, and some form of pollution insurance if hauling regulated waste.
3) If applicable, a DOT Hazardous Materials Certificate of Registration with the company address that has not expired. If designated as a non-regulated hauler only, provide hauler permit with company address that is not expired.
