STATEMENT OF WORK (SOW)
Maintain Airfield Markings and Rubber Removal
Mountain Home AFB, ID Capital Project No. 1036473 (QYZH 17-0155)
Invitation for Bid Design-Build 22 May 2020
PROJECT DESCRIPTION
1. Design and install new airfield pavement markings to comply with UFC 3-260-04, Airfield and Heliport Marking (16 May 2018), as well as remove accumulated rubber on the runway. The work includes:
a. Remove existing runway pavement markings
b. Replace runway pavement markings per UFC 3-260-01
c. Repaint taxiway edge markings
d. Remove rubber from the existing pavement at each end of the runway
DESCRIPTION OF WORK
1. Remove all existing markings from the runway, to include the overruns at each end. The only exception are the deceptive markings; these shall not be removed. The markings shall be removed to the hold line at each taxiway.
2. Install new pavement markings on the runway per UFC 3-260-04, 16 May 2018. The deceptive markings shall not be repainted.
3. Markings on asphalt pavement do not require a black border.
4. Repaint the taxiway edge marking at Taxiway Alpha and Taxiway Echo. These markings do not need to be removed prior to new markings being installed.
5. Accumulated rubber on the surface at each end of the runway shall be removed. Rubber shall be removed at the 30 end, starting at the end of the side stripes and continuing 3000 feet. Rubber shall be removed at the 12 end, starting at the end of the side stripes and continuing 2500 feet. Rubber shall be removed 35 feet on each side of the center line, for a total of 70 feet wide. Sandblasting or shot blasting shall not be permitted.
6. All work on the runway shall be conducted during non-flying hours/days to minimize impact to flying operations, unless approved by the Contracting Officer and Airfield Manager. The runway is typically closed from 1800 Friday to 0700 Monday. There will also be opportunities to work at night after flying operations have ceased for the day or between night flying operations. All other work on the airfield shall be coordinated at least seven (7) days in advance with the Airfield Manager to allow for current operations to be adjusted as necessary.
7. The Contractor is required to adhere to all airfield access and driving requirements.
a. Contractor personnel shall be allowed access onto the flight line via an Entry Authority List (EAL). This EAL is for flight line access only and is separate from the EAL required to access Mountain Home AFB. The EAL must be approved prior to flight line access.
b. Prior to operating a vehicle on the airfield, all drivers must receive Airfield Driving Training and pass the Airfield Driving Test. This must be scheduled at least 48 hours in advance.
8. At the end of each work day or when the runway reopens on Monday morning, all pavement markings shall be completed such that all required pavement markings for aircraft traffic are in-place. All airfield markings shall be complete and intact such as not to interfere with aircraft traffic.
9. The Contractor shall ensure all debris/trash, materials, equipment, vehicles, etc. are removed from the airfield after each day’s work and does not pose a Foreign Object Damage (FOD) hazard. The Contractor shall ensuring affected active airfield surfaces remain FOD free at all times. The Contractor shall utilize a vacuum type sweeper every time a construction vehicle or convoy traverses active airfield pavement to achieve a FOD free pavement. The Contractor shall perform frequent FOD sweeps to ensure haul routes remain clean at all times.
10. In addition to this SOW, drawings have been provided showing the exiting runway and taxiway markings to be removed and/or repainted.
11. The total Period of Performance (POP) shall be 120 calendar days. This includes both design and construction. The Contractor shall provide a detailed schedule showing how the POP shall be met.
GENERAL REQUIREMENTS
1. Work shall include furnishing all materials, labor, plant, tools, equipment, transportation, supervision, management, and other services (including engineering/technical design support), items and incidentals necessary to complete this project.
2. Any estimated quantities provided in project documents are considered informational only. The Contractor must validate quantities and all features of work necessary to deliver a complete and functional project.
3. The Contractor shall thoroughly review the project request for constructability and advise the Government of possible problem areas prior to submitting a proposal.
4. As-Built Verification: The Government does not guarantee the accuracy or adequacy of the as-built record drawings. It is the Contractor’s responsibility to field verify all Government provided record drawings prior to submitting a proposal. The Contractor may make multiple site visits, conduct exploratory excavation(s) with an approved Work Clearance Request (AF Form 103), or other intrusive inspection(s) upon approval of the Contracting Officer, and shall restore all disturbed areas to existing conditions. All exploratory investigations shall be at no additional cost to the Government and are considered field overhead. The Contractor is responsible for the accuracy of all measurements whether they are obtained in the field, or as existing drawing take-offs to develop accurate working drawings. All plans, labor, equipment, materials, and incidentals shall be furnished by the Contractor, unless otherwise specified.
5. Hazardous Materials to include, but not be limited to, Asbestos Containing Materials (ACM), lead based paint (LBP), mold, and mercury: The Contractor’s proposal shall include removal and dispose of any known hazardous materials in accordance with State and Federal rules and regulations and survey to determine if any additional ACM, LBP, mold, mercury or other hazardous material are contained in the work area. The Contractor shall perform an asbestos survey, mold survey, and LBP survey prior to the demolition, repair, and alteration and new construction. This information shall be reviewed and a Hazardous Materials Abatement Plan shall be produced to address these hazardous materials, if found.
6. The Contractor shall provide timelines and critical path items showing an understanding of the project requirements and capability in achieving the desired period of performance provided in this SOW.
7. The Contractor shall submit for approval all work schedules to the Government before start of work, unless otherwise specified. This shall be accomplished on and in accordance with instructions pertaining to AF Form 3064, Contract Progress Schedule, and AF Form 3065, Contract Progress Report.
8. If the Contractor chooses to have storage or facilities on MHAFB, the Government will provide an area within reasonable distance of utilities. The Contractor shall restore the land to its original condition within thirty (30) days of contract completion. If the Contractor chooses to have facilities on MHAFB, the Contractor shall be responsible for any costs associated with their facilities to include mobilization, demobilization, trailers, skirting, utility lines, connections and site restoration. Contractor on-site offices shall comply with handicapped accessibility laws and regulations and be in good repair.
9. Utilities: Subject to available supply, the Government will furnish reasonable amounts of electricity, gas, water and sanitary sewer capacity from existing outlets and supplies, without charge to the Contractor. The Contractor shall conserve the utilities. The Government, at their discretion, can charge the Contractor for utilities if the Contractor is using poor conservation practices. The Contractor shall install, maintain and remove any temporary connections and distribution lines at their own expense. If temporary lines are associated with this project, they shall be removed prior to the final acceptance meeting.
Communications access may not be available to the Contractor and the Contractor may be required to use wireless communications.
10. The Contractor shall secure all supplies and equipment stored on the project site to discourage theft or damage. Contractor shall maintain the project site in a clean, safe and orderly manner. Protection and security for materials and equipment on site is the sole responsibility of the Contractor.
12. If the work site is located in a restricted or controlled area, the Contractor may experience delays due to compliance with entrance/exit requirements of the restricted/controlled areas. The Contractor shall not be compensated for reasonable delays in accessing these restricted/controlled areas. The Contractor may also be requested to leave a restricted or controlled area at any time. The Contractor shall not be compensated for reasonable delays due to these security measures. The Contracting Officer may extend the contract period of performance when there is a change in security designations that limit or diminish Contractor access to a work site after commencement of the project. The Contracting Officer may also extend the period of performance of this project to offset undue delays, singular or cumulative, due to Contractor's compliance with security restrictions.
13. A burrowing owl and ground nesting bird survey is required prior to the start of construction. Do not disturb ground nesting birds; burrowing owls are protected under the Migratory Bird Treaty Act. Call 366 A7IE, Natural Resources Program Manager at
(208) 828-1784 or (208) 828-6351, for survey or assistance with wildlife/bird conflict issues. A burrowing owl and ground nesting bird survey WILL NOT be required because the work will be completed on the airfield runway.
DESIGN REQUIREMENTS
1. Survey Requirements: Topographic and/or Cadastral Surveys: Surveys/work as required for siting, grading, computation of quantities for excavation and fill, development of a particular project site, and the establishment of property lines in connection with a project shall be performed using conventional or other methods, such as a total station or Global Positioning Systems (GPS) for field data collection. All Survey data collected shall be provided to the Government in a digital format on CD, DVD, or Blue-Ray Disc with an attached Survey Report identifying survey method, equipment list, calibration documentation, survey layout, description of control points, control diagrams, and field survey data.
2. Design Review Meeting Requirements: Design review meetings shall be held within 14 calendar days after receipt of each design submittal. All comments will be documented by the Contractor and each item resolved at the meeting. A list of attendees and minutes of the meeting shall be taken by the Contractor and copies shall be forwarded to each attendee.
3. Design Analysis Requirements: Design Analysis shall include calculations, general description, history, assumptions, references, analysis of alternatives, energy conservation analysis/life cycle cost analysis, and code references, as applicable for each discipline.
a. The Contractor shall complete and submit a Constructability Review Checklist at Preliminary, Intermediate, Pre-Final, and Final Design states.
b. The Contractor shall provide three (3) sets of hard copy design analyses and one
(1) set of Adobe PDF electronic files to the Contracting Officer at each required design level.
c. Professional certification: At the Final Design (100% Design), the Final Design Analysis cover sheet shall have professional certification stamps.
4. Specification Requirements: Provide Contractor-originated Unified Facilities Guide Specifications (UFGS) that, in conjunction with the drawings, demonstrates compliance with materials, equipment, execution, and field quality control requirements of this project.
a. Modify the guide specification to fit the project.
b. Delete portions of the guide specification that cover work not included in the project.
c. When portions of the work involved are not covered in a guide specification, add requirements to the project specification, as necessary, using language and format similar to that employed in the guide specification.
d. Use guide specifications only as source documents and do not reference them in project specifications.
e. Do not combine work covered by various UFGS into one section unless the work to be incorporated is of a minor nature.
f. The specified products, equipment, fixtures, devices, and systems submitted by the Contractor and accepted or approved by the Government shall be used to construct the project.
g. Any applicable manufacturer or industry specifications associated with material, supplies, or systems shall be utilized. If a discrepancy exists between these documents, the Contractor shall bring this to the attention of the Government and shall utilize the most stringent requirement(s) in the preparation of the project design.
h. The Contractor shall provide three (3) sets of hard copy specifications and one (1) set of Adobe PDF electronic files to the Contracting Officer at each required design level.
i. Professional certification: At the Final Design (100% Design), the final specification cover sheet shall have professional certification stamps.
5. Drawing Requirements:
a. Basic design criteria utilized for each discipline will be included on the appropriate drawings as required by this project. Examples would include wind and live/dead loads assumed for structural elements, function specific conditions such as lighting levels or future use considerations for electrical systems, site specific climatic design conditions for mechanical systems, and International Building Code analysis.
b. Each sheet shall have a professional certification seal at Final Design.
c. The Contractor shall provide three (3) sets of Standard B size drawings in correct scale, one (1) set of Adobe PDF electronic files and one (1) set of Autodesk AutoCAD electronic drawing files to the CO at each required design level.
DESIGN SUBMITTALS
1. The Contractor shall provide the following design packages:
a. Intermediate Design (60% Design): The intermediate design includes complete site and landscaping plans; complete floor plans, roof plans, structural plans, elevations, building sections, wall sections, mechanical plans, fire protection plans, plumbing plans, communication plans, and electrical plans with details, notes, material, and equipment schedules; specifications in rough draft; and a design analysis that includes design narrative and analysis. The design analysis shall include all design assumptions and calculations for all disciplines, code analysis by all disciplines, manufacturers’ material and equipment cut sheets for major equipment, fixtures; finishes; completed environmental permit request forms; sustainable development requirements; listing of Government furnished materials, equipment and property; construction schedule to include any work phases to accommodate Government operations; copies of previous design review comments and responses; previous meeting minutes; and documentation of any other communication between the Government and the Contractor that influenced the intermediate design.
b. 60% Design Review Comments: The Government will provide comments after receipt and review of the Contractor’s design submittal. The percentage of design increases by 5% after Government acceptance of the Contractors responses to the design review comments.
c. 60% Design Review Meeting: After the Contractor receives the Government design review comments, a meeting led by the Contractor shall be held between the Contractor’s design team and the Government to discuss the Government review comments prior to the Contractor’s submission of responses to the Government review comments.
d. Pre-Final Design (90% Design): The pre-final design shall be the result of the Contractor’s design efforts from 65% to 90%. Pre-Final Design begins after Government approval of the Contractor’s responses to the Government’s review comments on the Intermediate Design (60% Design) which sets project at 65% design complete. The pre-final design includes any changes necessary to comply with Government comments to the intermediate design, complete plans and specifications, final design analysis, and color boards and finishes. The design analysis includes design narrative and analysis, all design assumptions and calculations for all disciplines, code analysis by all disciplines, manufacturers’ material and equipment cut sheets for major equipment, fixtures; and finishes;
completed environmental permit request forms; sustainable development requirements; specifications in rough draft; listing of Government furnished materials; final construction scheduled to include any work phases to accommodate Government operations; copies of previous design review comments and responses; previous meeting minutes; and documentation of any other communication between the Government and the Contractor that influenced the intermediate design.
e. 90% Design Review Comments: The Government will provide comments after receipt and review of the Contractor’s design submittal. The percentage of design increases by 5% after Government acceptance of the Contractors responses to the design review comments.
f. 90% Design Review Meeting: After the Contractor receives the Government design review comments, a meeting led by the Contractor shall be held between the Contractor’s design team and the Government to discuss the Government review comments prior to the Contractor’s submission of responses to the Government review comments.
g. Final Design (100% Design): Final Design (100% Design): The final design shall be the result of the Contractor’s design efforts from 95% to 100%. Final Design begins after Government approval of the Contractor’s responses to the Government’s review comments on the Pre-Final Design (90% Design) which sets project at 95% design complete. The final design includes any changes necessary to comply with Government comments to the pre-final design, complete plans and specifications, final design analysis, and color boards and finishes. Upon Government approval of the final design and Government approval of the Contractor’s proposed construction start date, the Contractor can commence construction activities.
2. Professional Certifications: All life safety plans, fire suppression and alarm plans and pre-engineered structure plans shall be submitted with professional certification affixed, regardless of the design level requirements.
3. Material Submittals: The Contractor shall generate a Material Submittal Schedule. The specification section and paragraph number applicable to each material submittal item shall be added to the Material Submittal Schedule. Consecutively number each page of the schedule of submittals, e.g. “Page 1 of 1”. Use all line number rows on one page before starting another page. Ensure all items requiring submittals are annotated on the schedule of submittals with the anticipated submission date. The Contracting Officer will review the schedule of submittals to ensure all required submittals are annotated and track required submittals for this project.
MANDATORY CRITERIA AND STANDARDS
1. The Contractor’s design and construction shall comply with the following references (the most current version at project award):
a. Unified Facilities Guide Specifications (UFGS). The UFGS is available from the Whole Building Design Guide website at http://www.wbdg.org/ffc/dod.
b. Unified Facilities Criteria (UFC) and Facilities Criteria (FC). The UFCs require compliance with many industry standards to include, but not be limited to, International Building Code, International Mechanical Code, National Electric Code, and National Fire Protection Association Codes, International Fuel Gas Code and International Plumbing Code. See UFC 1-200-01, DoD Building Code (General Building Requirements). The UFCs are available from the Whole Building Design Guide website at http://www.wbdg.org/ffc/dod.
c. Latest Version MHAFB Installation Development and Design– Provided by 366 Civil Engineer Squadron (MHAFB Only). Standard can be located at https://www.wbdg.org/ffc/af‐afcec/installation‐facilities‐standards‐ifs.
d. Latest Version MHAFB Temporary Environmental Controls Specification 01 57 19, dated 28 June 2018 (MHAFB Only).

l. OSHA Construction Industry Standards (29 CFR 1926). 29 CFR 1926 is included as part of OSHA Publication 2207 which combines both Parts of 1910 and 1926 as they relate to construction safety and health. They are for sale by the Superintendent of Documents, U.S. Government Printing Office, Washington, DC 20402 (MHAFB Only).
m. National Emission Standards for Hazardous Air Pollutants (40 CFR, Part 61) (MHAFB Only).
n. Asbestos Containing Material or any Class 1 ozone depleting chemical compounds are prohibited. Any asbestos material or Class 1 ozone depleting chemical compounds installed by the Contractor shall be removed and disposed of by the Contractor at the Contractor’s expense and shall not be subject to a claim against the Government (MHAFB Only).
o. Hazardous Materials: Refer to hazardous and toxic materials/substances included in Subparts H and Z of 29 CFR 1910 and to others as additionally defined in Fed.
Std. 313. (Those most commonly encountered include asbestos, lead paint, polychlorinated biphenyls (PCBs), explosives, and radioactive material, but may include others. The most likely products to contain asbestos are sprayed-on fire proofing, insulation, boiler lagging, pipe covering floor tile and carpet mastic, roof mastic, and drywall mud.) (MHAFB Only).
p. U.S. Army Corps of Engineers, Engineer Research and Development Center (ERDC), CAD/BIM Technology Center for facilities, infrastructure, and environment, A/E/C CAD Standard. The A/E/C CAD Standard is available from the Whole Building Design Guide website at http://www.wbdg.org/ffc/dod (MHAFB Only).
2. All drawing shall be prepared using latest version of Autodesk AutoCAD computer aided drafting software. The AutoCAD drawing shall be independently referenced with all font and symbol set incorporated such that the documents may be readily opened by Government personnel (MHAFB Only).
3. Drawing sheets shall use standard English scales on either Standard D (24” x 36”) or Standard B (11” x 17”) paper size (MHAFB Only).
4. The MHAFB cover sheet and title blocks shall be used. Templates for these will be provided to the Contractor on disk in AutoCAD (MHAFB Only).
a. The title block shall contain project information, including but not limited to Project number, Project Name, and date (MHAFB Only).
5. Survey Requirements: Accuracy level in accordance with “Geospatial Positioning Accuracy Standards, Part 2: Standards for Geodetic Networks,” published by the Federal Geographic Data Committee (FGDC), dated July 1998. All geodetic data shall be collected using the coordinate and projection system – World Geodetic System (WGS) 84, Universal Transverse Mercator (UTM) 13 North ellipsoid height, and orthometric height (EGM96). Data may also be requested in Military Grid Reference System (MGRS) coordinates. The Contractor shall use survey grade GPS, at an accuracy level of +/- 2cm.
6. Geographic Information System (GIS): GIS data shall be provided in ESRI’s ArcGIS 10.x shapefile (SHP) format. All additional attribution for the data will be based on the Spatial Data Standards for Facilities, Infrastructure, and Environment (SDSFIE) release
3.1 data model.
a. Entity naming conventions, attribute fields, and domain names shall be collected in the format defined by the SDSFIE release 3.1, except where modified by the Government. For more information, go https://www.sdsfieonline.org/Home/Index.
CONSTRUCTION REQUIREMENTS
1. Red-Line Construction Drawings: During the progress of the work, the Contractor shall keep a careful record of all changes and corrections from the original design layouts shown on the drawings. These red-lined drawings will be maintained at the job site, available for inspection by the Government. Upon completion of the project the red-line drawings shall be used to develop the as-built drawings.
2. Professional Certifications: All life safety plans, fire suppression and alarm plans and pre-engineered structure plans shall be submitted with professional certification affixed, regardless of the design level requirements.
3. Seasonal Heating, Ventilating and Air Conditioning Requirements: The Contractor shall provide temporary heating or cooling to existing occupied facilities if the air conditioning system or heating system is shutdown to perform work. Any proposals to shut down heating or cooling without providing temporary service must be approved through the CO at least two (2) weeks prior to interruption of service.
4. The Contractor shall hold weekly progress meetings to discuss current status, future scheduled work and any coordination requirements.
5. Pre-Final and Final Inspections:
a. The Contractor shall request from the Contracting Officer a final acceptance meeting. The request shall be in writing (email is sufficient) at least three (3) working days prior to the meeting.
b. Meeting will be chaired by the Contracting Officer and attended by appropriate Government and Contractor personnel. It is mandatory the superintendent be in attendance.
c. If any Contractor personnel are still working on the project, the Contracting Officer will immediately cancel the final acceptance meeting and direct the Contractor to reschedule when the work is complete.
d. If during the final acceptance meeting it becomes obvious to the Contracting Officer that the project is not ready for final acceptance, the Contracting Officer will immediately cancel the final acceptance meeting and direct the Contractor to reschedule when the work is complete.
e. The Contractor will generate a punch list of deficiencies that must be completed prior to the Government taking beneficial occupancy of the facility. During the final acceptance meeting, the Contracting Officer will ask the Contractor how much time he/she requires to correct the deficiencies and both parties will come to an agreement on the number of days to correct the deficiencies. The Government will schedule a meeting to verify the deficiencies are corrected.
6. Government Training: If new equipment is being installed, the Contractor shall schedule and provide training sessions for Government employees.
7. Records Documents:
a. Prior to completion of the project, the Contractor shall update the original electronic as-built drawing files or the Contractor’s final electronic design drawings as applicable and provide the Government with a draft of the as-built drawings in Standard D size, on paper for review.
b. Upon Government approval of the draft as-built drawings, the Contractor shall provide one (1) set of Standard D size drawings in correct scale, one (1) set of
Adobe PDF electronic files and one (1) set of Autodesk AutoCAD electronic DWG files on CD or DVD.
c. The drawings shall indicate, in addition to all changes and corrections, the actual location of all subsurface utility lines. In order that the location of the utility lines and appurtenances may be determined in the event the surface openings or indicators become covered over or obscured, the as-built drawings shall be clearly defined to show the end of each run including each change in direction. This may be accomplished, by offset dimensions to two (2) permanently fixed surface features. Valves, splice boxes, and similar appurtenances shall be located by accurate dimensions along the utility run from a reference point. The Contractor shall record the depth (below the surface of each run), in addition to the size and type of conduit.
d. GIS Files: The Contractor shall provide to the Government an electronic copy of the site layout and site utility plan in ESRI’s GIS shapefile format on a CD or
DVD.
8. Operations and Maintenance Manuals:
a. The manuals shall contain complete instructions for operation, inspection, testing and maintenance of the system, and shall include part breakdown diagrams. If applicable, the manuals shall also contain complete wiring schematics and diagrams. The Contractor shall provide the Contracting Officer all paperwork that was shipped with or attached to the product or material including installation or maintenance instructions for the product or material, part number listings, care and/or cleaning instructions, removing and replacing information, warranty information, toll or toll-free numbers for information and operator manuals.
b. Prior to completion of the project, the Contractor shall provide the Government with a draft Operations and Maintenance Manual Set on paper for review.
c. Upon Government approval of the draft Operations and Maintenance Manual Set, the Contractor shall provide three (3) indexed and bound paper sets and one (1) set of Adobe PDF electronic files.
9. Transfer and Acceptance of Military Real Property (DD 1354): At completion of the project, the Contractor shall fill out and submit for approval a “Transfer and Acceptance of Military Real Property" (DD 1354). The DD 1354 form can be obtained at http://www.dtic.mil/whs/directives/forms/eforms/dd1354.pdf.
SAFETY REQUIREMENTS
1. In order to provide safety controls for protection to the life and health of employees and other persons, for prevention of damage to property, materials, supplies, and equipment, and for avoidance of work interruptions in the performance of this contract, the Contractor shall comply with all pertinent provisions of the U.S. Army Corps of Engineers Manual, EM 385-1-1, entitled “Safety and Health Requirements Manual,” as most recently amended at time of notice to proceed (NTP). The Contractor shall also take or cause to be taken such additional measures as the Contracting Officer may determine to be reasonably necessary for the purpose.
2. The Contractor shall comply with OSHA General Industry Safety and Health Standards (29 CFR 1910), Publication V2206 and OSHA Construction Industry Standards (29 CFR 1926).
3. The Contractor shall maintain an accurate record of and report the following: exposure data and all accidents resulting in death, traumatic injury, occupational disease, and damage to property, materials, supplies, and equipment incidental to the work performed under this contract. The Contractor shall submit records to the Contracting Officer in the manner and on the forms prescribed by the Contracting Officer.
4. The Contracting Officer will notify the Contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately take corrective action. Such notice, when delivered to the site of the work, shall be deemed sufficient for this purpose. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective action has been taken. No part of the time lost due to any such stop orders shall be made the subject of a claim for extension of time or for excess costs or damages by the Contractor.
5. The Contractor shall be responsible for the safety and health compliance of all subcontractors.
6. If applicable; No Fewer than five (5) working days prior to the requirement for welding, gas cutting or brazing, the Contractor shall request a signed AF Form 592, USAF Welding, Cutting, or Brazing Permit from the MHAFB Fire Department, Fire Prevention Section.
COORDINATION REQUIREMENTS
1. Coordination: Contractor shall coordinate construction activities with appointed Government representatives and facility occupants.
2. Excavation Requirements:
a. Work Clearance Request (SOW Atch 1): Before commencing work on this project, the Contractor shall prepare and coordinate an AF Form 103, Work Clearance Request, with drawings indicating where the existing subsurface improvements are in the area and specific excavation and trenching locations widths and depths. The Government will provide information on which offices on MHAFB must coordinate on the AF Form 103. The Contractor, in coordination with Civil Engineering shall hand carry the AF Form 103 to the coordinating offices and get their approval signatures. See 366 CES Office Instruction (OI) 32- 3003, dated 1 April 2016 (SOW Atch 2).
b. Form 103 shall be provided to the Government prior to the Contractor requesting utility locates. A copy of the completed AF Form 103 shall be on the project site for the duration of the project.
c. Utility Locates: The Contractor must submit the AF Form 103 request at least fifteen (15) working days prior to initiating excavation and trenching to arrange appointments to get utility locates at the site. The Contractor is responsible for the protection of any underground utilities and structures identified on the AF Form 103 or by utility locate within five (5) feet of the identified location. The Contractor is responsible to maintain the utility locates until the Contractor no longer requires them to complete the execution of the project. Should the Contractor damage any underground utilities or structures, they shall immediately notify the Base Civil Engineer Service Desk and the Contracting Officer. If the damaged item was identified on the AF Form 103 or by the utility locate, the Contractor shall repair the damage immediately and return the item to its original working condition at their own expense. If the damaged item was not identified on the AF Form 103 or by the utility locate, the Government will repair it at their expense.
3. Outages/Interruption to Service: At least fifteen (15) work days prior to the requested outage, connection or disconnection, the Contractor shall notify the Contracting Officer in writing of pending outages and their anticipated duration. Outage types are:
a. Electrical Systems: High voltage connections and disconnections from MHAFB electrical grid. Coordination is required with Government Exterior Electric personnel prior to all outages.
b. Heating, Ventilating and Air Conditioning (HVAC): Discontinuing heating or cooling without providing temporary service.
c. Energy Monitoring and Control Systems (EMCS): Activation or deactivation of any EMCSs.
d. Gas Systems: Only Government personnel shall perform operation of valves on gas mains.
e. Fire Protection Systems: Activation or deactivation of fire protection systems to include interruption of electrical power or water supply to the fire protection system. The Contractor, in coordination with Civil Engineering, shall also notify the Fire Department Service Desk at least fifteen work days prior to the interruption.
f. Security Alarm Systems: When a security alarm system must be activated or deactivated or when an interruption of the electrical power affects a security alarm system, the Contractor, in coordination with Civil Engineering, shall also notify the Fire Department Service Desk and the Security Police Service Desk at least fifteen (15) work days prior to the interruption.
g. Water Systems: Operation of valves on water mains shall be by Government personnel only.
h. Communication Systems: When a telephone, local area network or wide area network must be activated or deactivated, the Contractor shall coordinate with Civil Engineering at least 15 work days prior to the interruption.
i. Road closures: Traffic plans shall be included in the specifications when street closures will be required during construction or when traffic flow is affected by the construction. Closure notifications must be made at least fifteen (15) working days prior to a proposed minor closure. Closures of a more complex or comprehensive nature require an advance notification greater than fifteen (15) days. The required advance notification period for complex or comprehensive closures shall be established by the Contracting Officer and the Contractor shall abide by any longer period once it is established.
4. Inspections, Tests and Reports:
a. Any required inspections, tests, and reports made by the Contractor, subcontractors, specifically trained technicians, equipment manufacturers, and others as required by this project shall be furnished upon request to the Government.
b. The Contractor shall obtain on-site inspection review and permission from Civil Engineering before back filling any trenches, pouring of concrete or utility tie-ins.
The Contractor warrants that authorized Government Representatives shall have unimpeded access to examine the work site(s), staging area(s) and all other areas utilized by the Contractor in the performance of a contract for the purposes of accessing the Contractor's performance and for any other purpose deemed necessary by the Contracting Officer or authorized Government Representative.
ENVIRONMENTAL REQUIREMENTS
1. Protection of Water Resources: The Contractor shall not discharge stormwater from construction sites nor discharge excavation ground water to the sanitary sewer, storm drains, or to surface waters. If the water is noted or suspected of being contaminated, it may only be released to the storm drain system if the discharge is specifically permitted.
Obtain authorization in advance from the Contracting Officer and Environmental for any release of contaminated water. Discharge of hazardous substances will not be permitted under any circumstances. The National Pollutant Discharge Elimination System (NPDES) stormwater program requires operators of construction sites of greater than 1 acre to obtain authorization to discharge stormwater under an EPA NPDES Construction General Permit (CGP). The Contractor shall refer to Idaho DEQ's Catalog of Stormwater Best Management Practices (BMPs) for Cities and Counties (http://www.deq.idaho.gov/media/622263-Stormwater.pdf) and utilize the necessary/appropriate BMPs for controlling/preventing stormwater runoff.
2. Disposal of Waste:
a. All construction debris will be taken off base to a state-approved landfill.
b. All trucks containing loose debris shall ensure that upon leaving the site, the debris is loaded in such a manner that shall prevent dropping of materials on streets and conform to local ordinances/laws. Fasten suitable cover, such as a tarpaulin, over the load before entering surrounding streets. The Contractor shall be responsible for cleaning up any materials that fall from trucks and be held liable for any resulting damages. NOTE: Idaho Waste Systems (IWS) will no longer accept ANY liquid wastes from Mountain Home AFB.
c. The Contractor shall submit all trip tickets from the landfill facility to show all debris is being disposed of in an approved landfill and in accordance with all Federal and State requirements.
d. To the extent practicable, all scrap metal, cardboard, wood pallets and #1-2 plastic must be sent for reuse or recycling and will not be disposed of in a landfill.
Recyclable materials may be dropped off at the base Recycling Center or when coordinated in advance, the Recycling Center may assist with material pick-up.
3. Hazardous Material Usage: HAZMAT includes all items that are covered under the Emergency Planning and Community Right-to-Know Act (EPCRA), or defined by Fed Std 313, Paragraph 3.2, or other applicable federal or state reporting requirements;
covered under the OSHA HAZCOM Standard (29 CFR 1910.1200) or the OSHA Occupational Exposure to Hazardous Chemicals in Laboratories Standard (29 CFR 1910.1450); and Class I or Class II ozone-depleting substances (ODS). All chemicals utilized in this project must be submitted for HWPM review utilizing the MHAFB Contractors Hazardous Materials Reporting Form (SOW Atch 3). A Safety Data Sheet (SDS) must accompany each product listed on the MHAFB hazardous material authorization, additional information may be required upon request. No hazardous materials are to be brought onto MHAFB without receiving approval. At the end of each calendar month the Contractor must submit any changes to the quantity of hazardous materials stored on site, this includes materials that have been used or identified for disposal via MHAFB Hazardous Materials Monthly Usage Log (SOW Atch 4). All hazardous waste to be shipped off site for disposal must be authorized and coordinated with the MHAFB HWPM.
WARRANTY OF CONSTRUCTION
1. In addition to any other warranties in this contract, the Contractor warrants that work performed under this contract conforms to the contract requirements and is free of any defect in equipment, material, workmanship or contractor furnished design or that furnished by any subcontractor or supplier at any tier. This warranty shall not limit the Government’s rights under the Inspection and Acceptance clause of this contract with respect to latent defects, gross mistakes and/or fraud.
2. This warranty shall continue for a period of one (1) year from the date of final acceptance of the work.
a. If the Government takes beneficial occupancy of a portion of the project, prior to final acceptance of the entire project, the warranty for the portion occupied will commence on the date of its occupancy.
b. For all other portions not beneficially occupied, the warranty shall begin on the date of their beneficial occupancy, or final acceptance of the entire project, whichever occurs first.
c. In the event the Contractor’s warranty of this clause has expired, the Government may bring suit, at Government expense, to enforce a subcontractor’s, manufacturer’s, or supplier’s warranty.
3. The Contractor shall remedy, at the Contractor’s expense, any failure to conform or any defect. In addition, the Contractor shall remedy, at the Contractor’s expense, any damage to Government-owned or controlled real or personal property when that damage is the result of:
a. The failure by the Contractor, subcontractor, or supplier at any tier, to conform to contract requirements or,
b. Any defect of equipment, material, workmanship, or design furnished.
4. The Contractor shall restore any work damaged in fulfilling the terms and conditions of this clause. The Contractor’s warranty with respect to work repaired or replaced will run for one year from the date of repair or replacement.
5. The Contracting Officer shall notify the Contractor, in writing, within a reasonable time after the discovery of any failure, defect, or damage.
6. If the Contractor fails to remedy any failure, defect, or damage within a reasonable time after receipt of notice, the Government shall have the right to replace, repair, or otherwise remedy the failure, defect, or damage at the Contractor’s expense.
7. With respect to all warranties, express or implied, from subcontractors, manufacturers or suppliers for work performed and materials furnished under this contract, the Contractor shall:
a. Obtain all warranties that would be given in normal commercial practice
b. Require all warranties to be executed, in writing, for the benefit of the
Government if directed by the Contracting Officer, and enforce all warranties for the benefit of the Government if directed by the Contracting Officer. Emergency warranty calls shall not exceed a 4-hour response time, while routine warranty response time shall not exceed three (3) workdays.
c. In response to an emergency warranty call the Contractor shall inspect the warranty item(s) and begin repairs or preparations for repairs within four (4) hours.
d. The Contractor shall respond to routine warranty calls in the same manner as emergency warranty calls within three (3) working days.
8. Unless a defect is caused by the negligence of the Contractor, subcontractor, or supplier at any tier, the Contractor shall not be liable for the repair of any defects of material or design furnished by the Government or the repair of any damage that results from any defect in Government-furnished material or design.
9. Manufacturer or other warranties in excess of one year shall inure to the Government.
The Contractor shall execute assignment of any and all such warranties to the benefit of the Government.
