STATEMENT OF WORK

REQUEST FOR PROPOSAL

1.0 SCOPE AND OBJECTIVE:

1.1 Introduction

This Statement of Work (SOW) outlines the requirement for the contractor to provide labor, equipment and materials to disassembling, relocating, and reassembling Calibration equipment located at the 412TH Maintenance Group of approximately 200 pieces of equipment. This work is in support of the installation of new flooring at B1450. Reassembly and reinstallation of all items relocated will be accomplished within 7 days upon completion of the flooring project. The work to be accomplished includes but is not limited to the following: General Labor and Heavy Equipment Operations.

1.2 Scope of Work

The contractor shall provide the following service:

a. A team of movers to move items:

(95) standard work benches {(3) to be located in rm. 303 of B1600,

(52) to be stored in Mod 511 of B1600, and (40) to be located in B1407},

(45) eight ft. tall metal shelves {(3) to be located in rm. 303 of B1600,

(17) to be stored in Mod 511 in B1600,

(25) to be located in B1407}, (3) L shaped desks to be located in B1407,

(4) cubicles to be located in B1407,

(11) ULISTAS cabinets that range from 150 lbs. – 300 lbs. to be located in B1407,

(1) 700 lbs. and (1) 4300 lbs. dead weight stands to be located in B1407,

(1) Wattmeter console weighing 300 lbs. to be located in B1407,

(60) pcs. of misc. test equipment ranging from 50 – 75 lbs. to be located in B1407,

(6) rotating metal towers 7 ft. in height to be stored in Mod 511 in B1600,

(1) 1,600 lb. block of granite measuring 18” x 25” x 36” to be stored in Mod 511 in B1600,

(1) horizontal force press weighing 5,400 lbs. to be stored in Mod 511 in B1600,

(1) 800 lb. vertical press and (1) 1,000 lb. vertical press to be stored in Mod 511 in B1600.

1.3 GENERAL REQUIREMENTS

a. Contractor shall field verify all dimensions and equipment to be relocated within B1450 and to be moved to Buildings 1407 and 1600.

b. Contractor shall keep the area clean at all times and remove all waste in a timely manner.

c. Repair/Replace any items damaged by the contractor’s employees and/or any sub-contractors while working on this project.

d. All contractor personnel shall comply with all measures in the Environmental Requirements Checklist.

e. Contractor will accomplish these requirements within the normal operating hours of Monday through Friday, no earlier than 0700 and no later than 1730. Special circumstances may dictate after hours work if the mission of Edwards AFB deems it necessary.

f. The contractor shall notify the 412 TW/MXG Technical Representative Mr. John T.

Shartzer at 661-277-0728, in a timely manner concerning any issues that would adversel impact the contractor’s ability to satisfy the requirements of this SOW.

1.4 Procedural

a. All personnel shall wear the appropriate Personal Protection Equipment (PPE) while on the construction site. Failure to comply will result in removal from the construction site until the PPE is donned.

b. Contractor shall coordinate construction effort with facility and/or project manager to prevent any construction delays, interruptions of classified work within the facility and/or movement of personnel and/or equipment.

c. All workers driving on Edwards AFB Flightline will obtain a flightline driver’s license by attending and passing the appropriate training.

d. All workers for this project shall be cleared by the customers’ security and given a daily temporary workers badge before entering the customers’ controlled area.

e. Any waste or excess material resulting from any contract requirement shall be removed and disposed of at a proper disposal site off Edwards Air Force Base by the Contractor in strict accordance with applicable Federal, State, County and local laws.

1.5 Temporary Facilities and Temporary Controls

a. Contractor shall install temporary signage and/or barricades to redirect personnel from the construction area.

1.6 Execution and Closeout Requirements

a. Contractor shall safely disassemble and transport all identified items from B1450 to buildings 1407 and 1600.

b. Contractor will coordinate with the Site Support Personnel within B1600 to safely barricade and re-route pedestrian traffic during the mobilization and demobilization of the precision equipment.

c. Contractor will safely return and reinstall precision equipment to B1450 and place it in its designated locations once the new flooring has been installed.
