Criteria Descrip�on

Previous Experience

Offerors must provide informa�on demonstra�ng that they have successfully performed hospital construc�on as a prime contractor or sub-contractor within the last five (5) years* where at least three (3), no more than five (5) construc�on projects where mul�ple trades were involved. Project magnitude shall be the price range of $2,000 and $1 million. The following informa�on is required:

• Government or Company which awarded contract/project

• Contract number if Government award

• Project Name

• Project loca�on

• Primary Point of Contact i.e., name, �tle, telephone number and email address

• Award amount

• Performance Period

• Descrip�on of Project i.e., scope of work

• Number of change orders/modifica�ons and type of modifica�ons (owner directed, differing site condi�ons, contractor requested) *The five (5) year �meframe is from award date of the project.

Past Performance Ra�ng

Past performance on contracts with Government agencies and private industry in terms of cost control, quality of work, and compliance with performance schedules. The past performance will be evaluated for all projects provided in sec�on F to include a review of Contractor Performance Assessment Ra�ng System (CPARS). The following informa�on is required for all projects noted in sec�on F: contract/task order number, project �tle, prime firm, start date, and comple�on date. Evalua�ons may also include addi�onal performance related from the firm, customer inquiries, Government databases, publicly available sources, and addi�onal projects in CPARS. Evalua�ons will consider superior performance ra�ngs on recently completed VA projects as well as the reputa�on and standing of the firm and its principal officials with respect to professional performance, general management, and coopera�veness. Also, consider, as appropriate, the record of significant claims against the firm because of improper or incomplete architectural and engineering services. Failure to provide requested data, accessible points of contact, or valid phone numbers could result in a firm being considered less qualified.

Safety & Infec�on Control

• The Offeror shall iden�fy personnel responsible for safety i.e., Competent and/or Qualified Person(s) and provide proof of competency/qualifica�on to meet OSHA Competent/Qualified Person(s) requirement.

• The Offeror shall outline how all workers whether a worker for the prime contractor or any subcontractor will be trained for the required minimum 10-hour OSHA training.

• Offeror shall provide CURRENT insurance Experience Modifica�on Rate and discuss their experience with on-the-job incidents or injuries during the last two calendar years.

• The Offeror should demonstrate knowledge of and experience with the American Society of Healthcare Engineer's (ASHE) Healthcare Contractor Cer�fica�on Program. Offerors with ASHE Healthcare Contractor's Cer�fica�on will be rated higher than others. At a minimum, the Offeror should describe familiarity with Healthcare Construc�on, along with applicable JCAHO standards, AIA guidelines, ASHRAE guidelines, EPA IAQ guidelines, and CDC guidelines.

Key Personnel • Project Manager, on-the job Superintendent seed project(s) biographical data including name, company posi�on, years with company, work and educa�onal background, experience applicable to hospital construc�on.

• The Site Superintendent(s)/Project Manager(s) shall have a minimum of a high school diploma or equivalent, five (5) years’ experience in construc�on and proof of two (2) projects managed by them valued over $100,000.00 for a minimum of 50% of the dura�on of the project(s) in a hospital facility. Person(s) must be employed by the prime contractor and show evidence of employment or contract for employment.

• Offeror may list addi�onal key personnel such as Senior Project Manager, Quality Control Manager, or Safety Officer(s) if firm employs personnel in these roles and they are separate du�es or different individuals from the Site Superintendent/Project Manager.

Quality Control • The Offeror shall submit a Quality Assurance/Quality Control plan regarding submital review, construc�on management, closeout, processes and logical measures to ensure successful execu�on of the seed project(s).

• The Offeror shall outline techniques for work coordina�on with subcontractors.

Describe the mechanisms that are in place to monitor performance of subcontractors and guarantee performance.

• The Offeror shall outline sound measures to monitor performance of subcontractors and guarantee performance, to include appropriate correc�ve ac�ons, management tools, methods, and documenta�on to correct performance if necessary.

• The Offeror shall describe in detail the procedures u�lized that ensure all contractual requirements will be completed on �me to include writen documenta�on such as payrolls, daily logs, submitals, warran�es, payments, and training comple�on.

Evalua�on Ra�ngs Descrip�on

Ra�ng Descrip�on Excellent Indicates an excep�onal approach, all-inclusive understanding of the requirements, and contains strengths. Contains no deficiencies.

Good Exceeds requirements, indicates a comprehensive understanding of the requirements, and contains strengths.

Contains no deficiencies.

Acceptable Meets requirements and indicates an adequate understanding of the requirements. Contains no deficiencies although weaknesses may exist

Marginal Failed to meet one or more requirements and indicates an inadequate understanding of the requirements. Submission has one or more weaknesses, significant weaknesses, and/or deficiencies.

Unacceptable Does not meet any of the requirements and indicates no understanding of the requirements. Submission contains many weakness, significant weaknesses, and/or deficiencies.

Neutral
