Acceptable Performance Level (APL): The APL’s for the task listed in the “Standards of Performance” above are listed in the Performance Requirements Summary Table (PRST) of this contract.
Method of Surveillance: The method of surveillance will be 100% or random on all scheduled task(s) unless listed differently in the Performance Requirements Summary Table (PRST) of this PWS. The COR will determine if the performance of each task(s) receives a 100% or random inspection unless a specific method of inspection is listed in the Performance Requirements Summary Table (PRST) of this PWS. The surveillance may include a visual inspection performed by the COR during or upon completion of task(s) performance by the Contractor or a review of Contractor provided documentation of scheduled task(s) completion.
Frequency of Surveillance: The COR will perform inspections in accordance with paragraph 1.4 of the PWS and can observe the selected task(s) at any time he/she determines a need to inspect a task(s) or notices a deficiency. Based on observed deficiencies in Contractor performance, subsequent inspections may either be reduced or increased. At a minimum, the COR will perform an inspection and complete a “Government Inspection Report” (see Attachment 3 to this PWS) weekly. The Government will use either 100% inspection or Random Inspection on the tasks listed. There are no special procedures to be followed to initiate unscheduled inspections.
Evaluation Procedures: Results of each inspection will be documented on a “Government Inspection Report”. (See Attachment 3 to this PWS for a copy of the “Government Inspection Report”). A “pass”, “fail” or “N/A” rating will be assigned to each task inspected based on the Performance Criteria listed below. A description of the inspection and observed defects will be recorded. A “fail” rating may require contractor re-performance of task(s). If notified by the Government to re-perform the task, the Contractor has ten (10) days from notification to complete re-performance. DLA Strategic Materials may (at the Contracting Officer’s unilateral discretion) levy monetary deductions against the Contractor for “fail” ratings documented in the “Government Inspection Report” as listed in the Performance Requirements Summary Table (PRST) of this contract. See Attachment 1 to this PWS for a copy of the
PRST. The COR will ascertain whether documented deficiencies were corrected and processes implemented to ensure re-occurrence of the deficiency is prevented.
Performance Criteria:
1) Each “Standards of Performance” requirement may contain several tasks. Each requirement will be evaluated based on individual tasks.
2) At a minimum, the COR will inspect all “Standards of Performance” listed in the PRST. The Government will use either 100% inspection or Random Inspection on the tasks listed.
3) A “pass” rating will be assigned to each task inspected that met or exceeded the established Acceptable Performance Level (APL) for each “Standards of Performance” requirement evaluated.
4) A “fail” rating will be assigned to each task inspected that fell below the established Acceptable Performance Level (APL) for each “Standards of Performance” requirement evaluated. When a “fail” rating is assigned, all observed deficiencies must be documented on a “Government Inspection Report”. (See Attachment 3 to this PWS for a copy of the “Government Inspection Report”).
5) A “N/A” rating will be assigned when no work was required to be performed on a specific task.
Corrective Actions:
1) The COR will notify the Contractor within twenty-four (24) hours each time a “fail” rating is assigned and discuss the basis for the rating.
2) The COR will solicit the Contractor’s proposed corrective action(s) to preclude re-occurrence of the deficiency(s). The Contractor has ten (10) days to complete re-performance.
3) Based upon the Contractor’s performance, the COR may recommend corrective action(s).
Subsequent inspections may be performed more frequently if deficiencies continue.
4) The COR will document the results of each inspection using a “Government Inspection Report”.
(See Attachment 3 to this PWS for a copy of the “Government Inspection Report”). The COR will complete a “Government Inspection Report” each month. The “Government Inspection Reports” will be maintained on site in the “COR File” and forwarded to the Contracting Officer upon request.
