About this file
This is an Offeror Self-Performance & Subcontracting Disclosure Checklist template designed to ensure compliance with FAR 52.219-14, which establishes limitations on subcontracting for small business contracts.
The checklist requires offerors to disclose their performance approach by indicating whether the prime contractor will self-perform required work or utilize subcontractors. For primes electing to subcontract, the form mandates identification of each subcontractor including legal business name, Unique Entity ID, assigned NAICS code, and size representation under that NAICS code. Subcontractors must represent their eligibility as "similarly situated entities" by confirming small business status and declaring any applicable program certifications such as Service-Disabled Veteran-Owned Small Business (SDVOSB), Women-Owned Small Business (WOSB), Economically Disadvantaged Women-Owned Small Business (EDWOSB), HUBZone, or 8(a) certification. The prime contractor must certify that it will not subcontract more than the allowable percentage of work to non-similarly situated firms, that all listed subcontractors have provided accurate representations, and that it understands compliance failures may result in termination for default, negative Contractor Performance Assessment Reporting System (CPARS) ratings, or other government remedies. An authorized official must sign and date the certification, and offerors may provide optional clarifying remarks regarding their performance strategy.
