Attachment A - SOW:
3.4 - Medication Management
Can clarifying language please be added to the Adult Medication Treatment Regimen? Proposed langauge included in red Italics: "The NHDP uses a different treatment regimen than the World Health Organization (WHO), see https://www.who.int/teams/control-of-neglected-tropical-diseases/leprosy/treatment. The following recommendations are based on the NHDP recommendations but the contractor is not bound to this guideline and may select a regimen substantiated by their clinical expertise and patient-specific factors ."
HD treatment guidelines provided in the SOW are the current recommendations from NHDP. The contractor is allowed to treat a patient according to its best practices and patient comfort level.
Attachment A - SOW:
Task 6. Third Party Reimbursement or Medical Co-payments
States "Medical co-payments and other remaining payments shall be paid by the contractor for all provider approved HD-related care without regard to the patient’s ability to pay" and goes on to cap reimbursement at $5,000. We have seen a significant increase in the number of new patients over the past few years, including quite a few who are out-of-state and therefore not eligible for financial assistance and us unable to assist with enrolling in their state's Medicaid. Can the cap language please be removed or clarification included on the cap being negotiable if the patient volumes and lack of coverage options justify it?
NHDP will not remove the $5,000 cap for this cost reimbursement line item. The contractor may request an increase in this line item from the Contracting Officer during the life of the contract if the justification and patient data supports the increase. All costs must be approved by a Contracting Officer in writing PRIOR to incurring the cost.
19 none identified Budget for RFP-can we submit budget to reflect the 4 ½ years or should it be for Option Year 1?
Proposals shall include a budget for each line item in Section B.2, Compensation Cost/Price, including the entire period of performance (base period of 6 months; 4 option periods of 12 months each and a 6 month extension option). Please see Section B for the price/cost breakdowns required. The total period of performance for all periods (if exercised) will be 5 years.

Task 3.1 Eligibility for Service – Can the following verbiage be added that patient referrals should be made by a physician or licensed healthcare professional prior to coming in for health care services?
No, NHDP does not plan on changing this language in the SOW. Ruling out HD as a diagnosis is a required task (see Task 3.1).
24 none identified 12 of 40 Task 5.1 Patient Travel Reimbursement - Will the NHDP provide screening questions to help determine patient need and financial ability for travel and hotel reimbursement?
NHDP is currently developing a social determinants of health screening tool that can be used to assess patient needs. This tool will be posted on the NHDP Website when available.
The contractor is encouraged to use screening tools that are widely available online.
25 none identified

Local, state, or federal programs for disability include SSDI (federal); public housing, rental assistance, subsidized housing, disability vouchers, job training, healthcare program (to include Medicaid), educational programs -https://www.usa.gov/disability-programs

The words, "task order," were deleted from L7. No IDIQ contract is contemplated. Contract will be stand alone contracts.
33 Section L.7.5 49 Goals are outlined for all small business types, excluding veteran-owned; what is the percentage requirement for VOSB?
L.7.5 was revised. Veteran-Owned Small Business (VOSB) was deleted. The only veteran category is Service-Disabled Veteran-Owned Small Business. Also, section L.7.3 was revised to correspond with L.7.5.
34 Overall Is there a template that can be used to respond to this RFP? If not, should we use the same format from our 2018 submission?
Please follow the instructions in the Solicitation and ensure that your proposal contains all required information and documentation. The 2018 submission may have had different requirements.
Do we need to create a subcontracting plan for an award less than $750k
No, a subcontracting plan is not required if the total amount of the award including the Base Period, Option Periods 1-4, and 6 month extension will be less than $750,000.
36 Attachment A, 3.3 4
3.3 Laboratory tests show that a baseline QuantiFERON-TB Gold is a required. Can an alternate IGRA (i.e, Tspot) be utlized instead of QFT?
Yes
37 Attachment A, 3.4 5
Solicitation states that immunologically competent patients should be treated with the NHDP treatment guidelines and the regimen table shows the once-montly RMM regimen. Please clarify if this regimen is required, or if HD clinicans may consider other regimens as per the NHDP Manual?
See response to question #17
38 Attachment A, 3.7 7
Community Outreach and Education: the solicitation requires the offeror provide monthly education activities that "go beyond the normal practice of informing patients about their health…" Please clarify what that looks like for a HD clinic that may see 10-12 patients/year. This requirement appears to be in addition to the two community outreach activities required each year, so the question is what does monthly reporting of patient education beyond routine patient education look like?
What is a reasonable number of hours is recommended?
Examples of Community Outreach and Education have been provided in the solicitation. The contractor may use its best judgement to determine method and length of activities based on patient needs and contractor capabilities.
39 Attachment A, 9 13 Will the reporting system be a NBS (NEDSS -based system)?
No, the NHDP reporting system is an Electronic Health Record (EHR) that is under development. NHDP is currently exploring a data use agreement with CDC that may be used to lower the burden of reporting requirements in the future.
40 Attachment A, 9 13 Are sub-contractors required to have access to the reporting system?
Subcontractors who perform any of the reportable tasks in the SOW will need to access the NHDP reporting system.
41 Attachment A, 9 13
Under Task 9, reporting requirements include submitting encounters and surveillance forms to the NHDP using the Reporting Website. Please clarify reporting method. Are reports to be submitted using the Hansen's Disease Surveillance form?
Can you clarify if the Access Database will be required for reporting bi-monthly or if that requirement will change? Will Access database still be used?
The Surveillance form is a separate deliverable from the reports. NHDP is developing a new reporting tool that will replace the Access database. However, the current Access database bimonthly reporting tool may be required until the new reporting tool is operational.
42 Section L 43
For the technical approach, it requries that the offeror provide a copy of the facilities' most recent healthcare survey, copy on an approved State Application for Licensure, a copy of the certification demonstrating…accreditation. Are these 3 separate documets or will a letter by Joint Commission stating the certification suffice?
A letter from Joint Commission is sufficient.
43 Section F.5 11 & 12
The table outlines all reporting activities. Should the Hansen's Disease Surveillance Form, NHDP Annual Follow-up Form, etc.
include on the first page of each submission, 1) Contract number
2) Deliverable item number
3) Deliverable item delivery due date
4) Date of submission
Yes, deliverables should identify the contract information.
The information may be added to the subject line of the email. When the new reporting system is operational, vendors will be instructed on the new reporting process.
44 Section F.5 13 On page 13, it states, “Deliverables for Optional Tasks, (if exercised). What does, “if exercised" mean?
Please disregard since the Optional tasks were deleted from the SOW (see revised SOW).
45 Section F.5 13
For optional task, number 2 “Optional Task 1, Position Sensitivity Designations: background investigation(s) which is applied to one employee". What do you mean by background investigation? What sources should be used to do a background investigation?
Background checks and HRSA's personnel security systems, policies, and procedures seek to manage the risk of staff (permanent, temporary, or contract staff) exploiting, or intending to exploit, their legitimate access to an organization's assets or premises for unauthorized purposes.
This is done by conducting background investigations. HRSA conducts the background investigations. The contractors need to support the process and ensure their staff complete the process in a timely manner.
46 Section F.5 13
Clarification is needed on number 5, optional task 3 – unclear what is meant regarding Security requirements for GOCO and COCO – these are acronyms used throughout the document without an explanation. This row further states that security requirements for GOCO and COCO resources assessment and authorization package should be completed at least 30 days prior to the operational readiness review (ORR). What is an ORR? When is this typically done?
Please disregard. The Optional Tasks have been removed from this contract. Please see revised Statement of Work
(SOW).
47 Section F.5 13 For number 6, optional task 3, does asset management refer to IT hardware and software purchased using NHDP funds?

Please disregard. The Optional Tasks have been removed from this contract. Please see revised Statement of Work
(SOW).
51 Section F.5 13 Is number 12, optional task 4, Agency assessment and authorization package applicable to Texas?
Please disregard. The Optional Tasks have been removed from this contract. Please see revised Statement of Work
(SOW).
52 Section F.5 13 For number 14, optional task 4.6 please describe what is an Incident and Breach Response Plan (IRP)
Please disregard. The Optional Tasks have been removed from this contract. Please see revised Statement of Work
(SOW).
53 Section H.5 (d) 20 Medical liability insurance: Can NHDP funds support a portion of this cost if included in the application?
The NHDP will not provide funding for medical liability insurance.
54 Section H.5 (d) 20 Medical liability insurance: If an agency is exempt, will the offeror be ineligible for NHDP funding?
Not enough information provided. The Offeror should explain and provide documentation of the exemption in their proposal so the Technical Evaluation Panel can evaluate.
55 L.1.2. 41
In L.1.2., it states volume 1 should include a statement indicating whether or not any exceptions are taken to the terms and conditions of the RFP as part of the proposal letter. Do you mean the statement should be stated in Vol 1 and also in the proposal letter?
Yes.
56 L.4.1 42 It states the proposal must stipulate that it is predicated upon all terms and conditions of the RFP. Question for NHDP: Should this be included in the proposal letter or only in volume 1?
Include the statement in the letter and vol. 1.
57 L.4.1 42 It states the proposal is firm for a period of at least 180 days.
Question: should this statement also be repeated in the proposal letter or only in volume 1?
Include the statement in the letter and vol. 1.
58 L.4.B.1 43
The proposal indicates, the most recent healthcare survey, Can you provide more clarification about the survey such as should the requestor be an outside entity? Do you have template of the type of questions that should be answered on a survey? I’m not quite clear on the ask.
The healthcare survey is based on the accrediting body that completes their facility’s survey, i.e. Joint Commission, AAAHC, etc.. The surveyor will verify any licensure, which in such case a letter of accreditation would be sufficient. No template of questons is available.
59 L.4.B.1 43 For facilities and equipment, Do you want all of the bulleted items? Or are there specific documents that are preferred?
Please respond to all of the bulleted items.
60 L.4.D 45 For L.4.D, second bullet it states include professional persons who will spend significant time on the contract. Question to NHDP: Please quantify significant time?
"Significant" was deleted from L.4.D. Please identify all professional persons who will be working on the contract.
61 L.5 46
For volume II – Section 508/VPAT – questions requested on the checklist is very technical, what support will NHDP provide to guide offerors how to present this information to their contracted entity. Also, is the checklist the only requirement for Volume II?
Offerors are expected to be able to prepare a proposal that meets the requirements in the Solicitation. Section 508/VPAT requirements are a part of this competitive requirement and will be evaluated according to Section M.6.
62 L.6 46 When submitting the CPARS, how do we access past assessment reports in sams.gov or should I use my saved copies from sams.gov?
SAM.gov has links to Customer Service at the bottom right of their website.
63 M 4.2 53
In section B, #2 it states to "describe the services which will need to be subcontracted to fulfill Task 2 of the SOW". Do you mean Task 3? If not, please elaborate on what you need to know regarding Task 2, as Task 2 is well-described regarding the kick-off meeting.
This evaluation criterion was revised to say Task 3.
64 General Question #1 Will all questions received from the applicants be combined and a response submitted to everyone?
Yes.
65 General Question #2 How will applicants be notified about the status of their application? Will it be posted in sams.gov? Or will an email be sent to the applicant?
Awards are posted on SAM.gov. Vendors submitting a proposal will be notified via email.
66 General Question #3 A page limit was not set for Volumes III & IV. I'm thinking these are primarily template driven, therefore, a page limit may not be needed. Is this assumption correct?
There is no page limit for volumes III and IV.
67 General Question #4 Can we be provided a fillable format of the attachments A-I?
If available and appropriate, fillable forms are uploaded to SAM.gov.

Compliance Checklist/VPAT – One of the requirements of the RFP is the Section 508. In our case that we are providing medical services to HD patients, what is the connection? Is this focused on employees? In our actual contract, it was not a requirement for us when we submitted the HD 2018-2023 RFP.

I am wondering if you have any suggestions on who should be listed as the Project Director? Our doctor retired and we have a new doctor starting in May/June 2023. Since the doctor is not currently on board yet, should we list someone else as the Project Director, perhaps our Department head or faculty that is covering the Hansen's clinic? Additionally, should the new doctor be added to the proposal now? or should she be added once she is on boarded?
We recommend including current and future staff that meet the evaluation criteria.
84 Cost Template Do you have the cost proosal template for the Hansen's disease solicitation in excel formate or do I need to fill it out in PDF?
No, the template is provided as a general guide to the cost elements needed.

We are working on the Compensation Cost/Price: Should the hourly rate be based on actual/current hourly rate of the key personnel? Or should be a county hourly rate?
Please see Section L.7 on business proposal instructions. The Offeror must submit a cost proposal fully supported by cost and pricing data.

Should the Base Period of Performance and the Option Period One to Four be the same cost?
No, the base period includes 6 months of costs and Option Periods 1-4 are for 12 months each; and then a 6 months extension option period. Most Offerors include an estimated inflationary cost of living increase in the cost/price for option periods.
