About this file
This document is an Amendment (0002) to a Request for Quote (RFQ) for the Zuni Comprehensive Community Health Center Equipment Inventory Database, extending the proposal due date to June 06, 2025 at 03:00 PM (MT). The RFQ seeks a contractor to develop an internet database containing up-to-date information and Instructions for Use (IFUs) for medical, dental, surgical equipment, instruments, biomedical service manuals, and facilities manuals. Key details include: the database must operate via IPv6, be accessible only within the facility, accommodate multiple users with system admin and campus user access, and potentially include a mix of existing and newly sourced equipment manuals. The solicitation is set aside for Indian Small Business Economic Enterprise (ISBEE) under the Buy Indian Act, with a secondary consideration for other small businesses if ISBEE quotes are not reasonably priced.
The amendment includes a comprehensive question and answer section addressing technical requirements, user access, inventory gathering, and past performance assessments. Notable clarifications include: contractors can propose new database platforms that meet federal guidelines, every staff member will have login access, the health center does not have a complete existing inventory, and §508 conformance reports are to be determined. The solicitation allows joint ventures under SBA Mentor-Protégé regulations and requires submission of a technical approach, past performance documentation, price quote, Indian Economic Enterprise representation form, and signed SF18.

Section # Section Title Question Answer
3 B Contract Price
Is the approximate quantity of items included under the Schedule of Charges sections 001, 002, 003 and 004 available?
Includes all known IFUs for Services requested. Ie; All
Surgical Instruments, Dental instruments, Biomedical and Facilities manuals from ALL HOSPITAL equipment supply manufactures regardless of our inventory.
4 C SOW
“Purpose” references up-to-date info for OEM manuals.
Is there an existing database?
OEM manufactures keep their manuals updated. It will be up to the contractor to work with OEMs.
4 C SOW
Are there existing manuals already available that only have to be digitized and added to a database library?
Or should a contractor anticipate a mix of existing available OEM manuals and others that will need to be sought out?
The Contractor will need to anticipate a mix of existing available OEM manuals and others that will need to be sought out.
4 C SOW
Can a contractor propose and implement a different/new database platform (that operates via IPv6,), such as a vetted AI-software our team currently works with on other projects and finds easy to use, robust, and reliable? Or does the ZCHC have a specific database platform in mind? Our proposed solution would be self-contained to the facility, though could grant access to any user ZCCHC wanted (if needed.)
The Federal government has guidelines and implement IPv6 enabled solutions. A contractor can propose and implement a new database platform that operates via
IPv6, provided it will adhere to Federal policies and requirements to include technical support and training for the users. To correct and update- The functions to meet future IPV6 infrastructure.
4 C SOW
In description of services, guidance states the database will be for “ZCCHC departments and staff.”
Please confirm if there are or are not other intended users that would require external access, such as the Indian Health Service- Albuquerque Area Office, IHS Rockland, MD, HHS, or any other entity authorized by the ZCCHC?
Every staff member will be able to log in as user for the entire data base which will only be accessible at our facility. This requirement is for
Zuni Comprehensive Community Health center only.
4 C SOW
SOW states, “contractor shall provide access to an Internet Database that consists of Up-to-Date Information of Use for Medical, Dental, Surgical equipment and instruments, Biomed Service Manuals and Facilities Manuals.” Does this specifically include inventory the ZCCHC currently utilizes, and/or will this include other planned or needed OEM manuals and information (i.e. items added over the diff years of the contract that will have to be monitored or requests sent by ZCCHC?)
See first question.
4 C SOW
Is there an existing inventory list for the 4 sections, under the sections - Medical, Dental, Surgical equipment and instruments, Biomed Service Manuals and Facilities Manuals? Or a list of “volume” in categories so that a contractor can project labor time requirements for
We do not have an inventory of every piece of equipment or instrument.
Instruments like forceps, Dental drills etc:
implementing its database development?
4 C SOW
Our team would plan to gather an inventory of all items in the identified sections to be included in the data base development, to be most efficient, would contractor staff be allowed access to gather inventory information on site? Or is this included in working with the Joint Commission?
See first question.
5 C SOW
From “Database will have the capability to accommodate multiple users,” can a number of users be provided? This would help determine how to best coordinate and price the platform we would propose utilizing.
We do not need users access for each employee. System admin and Employee campus user is what we currently use for our SDS system.
5 C SOW
If a number of users can be provided, could users be assigned to specific sections only, or shall we anticipate all users would require full access to all sections of the database?
See above.
17-18 (& 33- 34)
10 (& 19, §d)
§508
ACCESSBILITY/WCAG
Please confirm §508 conformance reports and WCAG checklists are due at the end of each contract year. Or if this is TBD. The Solicitation appears to state both, required or if determined by CO office.
To be determined.
38 3 Past Performance
Assessments
Will the agency accept PP surveys from both entities in a Joint Venture that is under an SBA approved Mentor- Protégé, which is allowed under All Small SBA MPA JV regulations?
Yes.
