About this file
This is a Solicitation Survey form (W912WJ26QA171) that vendors must complete in full to submit responsive quotes. The form requires comprehensive company information including SAM CAGE Code, business type, and point of contact details, with explicit warning that incomplete submissions will be deemed non-responsive.
The survey mandates detailed disclosure of vendor experience, including years in service, prime and subcontractor contracting history, federal government contract experience, and any work completion failures. Vendors must identify key personnel assigned to the project with minimum and maximum employee counts, payroll status, and hiring needs. Section 6 requires specific information on each employee's position, title, and years of experience. The form specifies required certifications including First Aid/CPR, Competent Person in Fall Protection, Rope Access, applicable trade licenses, Crane Operator/Qualified Riggers credentials, and Boat Operator certifications. Vendors without completed certifications must provide detailed plans for obtaining them, including course names, providers, potential training dates, and course links. Subcontractor information requires the name, point of contact, and location; vague responses such as "to be determined" result in non-responsive ratings. Section 10 requires three past performance contract references completed within three years, with preference for government contracts. Each reference must include the contracting agency or company name, contract number, total value, complete POC information, dates, and detailed scope description sufficient for government evaluation. The certification section requires a signed statement that all information is complete and accurate.
