About this file
This is a Solicitation Survey form that vendors must complete and submit with their quotes to be deemed responsive and compliant with the solicitation requirements.
The survey requires vendors to provide comprehensive company information including name, SAM CAGE Code, address, and point of contact details, along with business type classification. All information submitted must pertain to the prime vendor submitting the quote, not subcontractors, with claims not substantiated by SAM registration subject to rejection. Vendors must document years of experience in the services, years as a prime or subcontractor, and years performing federal government contracts. Question 4 requires vendors to disclose any failures to complete awarded work, with vendors checked "No" but having associated terminations subject to removal from consideration. The survey mandates disclosure of key prime personnel availability, including minimum and maximum employee counts, payroll status, and anticipated staffing levels needed for performance.
Question 6 requires detailed employee experience information for all personnel performing work. Question 7 addresses certificates and licenses with specific focus on: First Aid/CPR, Competent Person in Fall Protection, Rope Access, applicable trades licenses, Crane Operator/Qualified Riggers qualifications, and three custom requirements marked with "X": Niagara N4 Certification, recent and relevant experience performing similar central plant controls migrations at federal or institutional campuses, and demonstrated experience migrating proprietary Niagara platforms to open-native platforms with specific Distech/Honeywell LON integration experience. Vendors must either attach current certifications or provide detailed plans for obtaining certifications including course names, providers, training dates, and course links. Questions 8 and 9 require subcontractor information with teaming arrangement documentation; vendors stating "to be determined" or "information provided upon request" are removed from consideration. Question 10 requires three contract references from the past three years with similar scope, size, and complexity, preferably government contracts, including complete contact and project scope details. All submissions must be certified by an authorized company official.
