About this file

This document is a Solicitation Survey template that vendors must complete to be considered responsive to a federal contract opportunity.

The survey requires vendors to provide comprehensive company information including legal entity type, SAM CAGE code, and primary point of contact. Vendors must certify that all information submitted pertains to the prime contractor only, not subcontractors, and that any claims (such as employee counts) are substantiated by SAM registration. The survey requests detailed experience data: total years in the specified services, contracting experience as prime or subcontractor, years performing federal government contracts, and disclosure of any failed contract performance. For personnel, vendors must identify minimum and maximum employee counts, confirm regular payroll status, and specify anticipated staffing levels needed for the project. Additionally, vendors must provide detailed information for each employee performing the work, including position, title, and years of experience. The survey requires documentation of specific certifications and licenses including First Aid/CPR, Fall Protection competency, Rope Access, applicable trade licenses, Crane Operator/Qualified Riggers, Boat Operator, Qualified Tree Worker documentation, and Level 2 SSHO certification, with vendors required to provide either current certificates or documented plans for obtaining certifications with course names, providers, training dates, and course links. For vendors using subcontractors, the survey mandates provision of subcontractor name, point of contact, and location, with teaming arrangement documentation attached; vendors stating information is "to be determined" or "available upon request" are deemed non-responsive. The survey requires three past performance references from contracts completed within the past three years, similar in nature, size, and complexity to the scope of work, with preference for government contracts. For each reference, vendors must provide contract agency or company name, contract number, total value, complete POC information, contract dates, and detailed scope description sufficient for government evaluation. A certification statement attesting to accuracy and completeness must be signed by an authorized company official.
