About this file
This is a Snow Plow Checklist and Service Evaluation document issued by the Department of Defense, Veterans and Emergency Management - Military Bureau for oversight of snow plowing and sanding services at the Bangor East Complex in Maine. The checklist establishes performance evaluation standards for contractor compliance, requiring a minimum of monthly periodic performance evaluation tours conducted by the Program Administrator or designee. The evaluation framework assesses contractor performance across general tasks including snow and ice removal from parking areas, roadways, entrances, gates, and sidewalks; treatment of surfaces at the time of plowing; clearance of designated areas by 6:00 am; return visits within 24 hours of precipitation completion; proper snow pile placement in specified areas only; and removal of contractor-created drifts from fire lanes, hydrants, and dumpster enclosures. Special requirements vary by facility location, including 24/7 operations at Bangor Flight Facility Building #260 and RTI #700, as well as shoveling services for walkways, handicap ramps, and stairs at RTI and Camp Keyes.
The evaluation process utilizes a rating system of Excellent, Good (Meets Expectations), Needs Improvement, and Not Applicable. A contractor receiving four or more negative responses on yes/no questions and Needs Improvement or Unacceptable ratings during an evaluation tour will be documented as poor performance, triggering a two-week correction period. Failure to resolve issues results in potential contract termination with 30 days' notice, and three consecutive poor performance evaluations result in automatic termination eligibility. Contractors may appeal termination decisions within 15 calendar days to the Director of the Division of Procurement Services. Additional documentation requirements include photos and detailed listings of pre-existing damage within 30 days of contract start, timely responses to communications within one business day, identification of vetted personnel, and provision of contact information for gated areas. The checklist designates Ken Merrifield at kenneth.r.merrifield.nfg@army.mil as the contact for completed evaluations.
