About this file
This is a Snow Plow Checklist and performance evaluation document used by the Department of Defense, Veterans and Emergency Management - Military Bureau for monitoring snowplowing and sanding services at Bangor Air Force Reserve Center (AFRC) in Maine. The checklist establishes performance standards for contractors providing snow removal and sanding services, with mandatory periodic evaluations occurring at minimum monthly intervals. Additional evaluations may be scheduled with three business days' written notice via email. The document outlines specific service requirements including clearing and sanding of designated areas when precipitation accumulates two inches or more, completion of parking lots, access roads, and sidewalks by 6:00 a.m., and response to special requests without interruption. Special facility requirements are identified, including 24/7 operations at Bangor Flight Facility Building #260 and Bangor RTI #700, which require on-demand snow removal notifications from facility point of contact, program administrator, or security guards. Additional requirements include shoveling of walkways, handicap ramps, and stairs at RTI and Camp Keyes locations.
Performance evaluation criteria are established with clear consequences for non-compliance. Contractors receiving four or more negative responses on yes/no questions or "Needs Improvement" ratings during evaluation tours are classified as poor performers and given two weeks to correct deficiencies; failure to comply results in contract termination with thirty days' notice. Three consecutive poor performance evaluations trigger automatic contract termination. Contractors have fifteen calendar days to appeal termination decisions to the Division of Procurement Services Director. Additional performance standards include snow pile placement in specified areas only, prevention of drift creation at fire lanes and hydrants, daytime-only snow pile movement requirements, and photo documentation of pre-existing damage within thirty days of contract start. All contractors must provide contact information for gated areas and respond to emails and phone calls within one business day. Completed evaluation sheets must be returned to the designated DFE contact, with attendance signatures required from facility contacts, DFE representatives, and vendor/provider contacts.
