About this file

This document is a Snow Plow Checklist and performance evaluation tool developed by the Department of Defense, Veterans and Emergency Management - Military Bureau for assessing contractor performance on snowplowing and sanding services at Camp Keyes in Maine. The checklist establishes a comprehensive evaluation framework requiring periodic performance evaluation tours at minimum monthly intervals, with additional evaluations scheduled upon written notice of at least three business days. The evaluation addresses general snow removal tasks including clearing and sanding of designated areas when precipitation accumulates two inches or more, response to special requests, maintenance of gate areas, treatment of roadways and parking lots, ice management, and clearance completion by 6:00 a.m. Special requirements are identified for specific facilities including Bangor Flight Facility Building #260 and Bangor RTI #700, which may operate 24/7 and require around-the-clock snow removal capabilities upon notification. Additional tasks include shoveling of walkways, handicap ramps, and stairs at specified locations.

Performance standards are clearly defined with specific consequences for non-compliance. Contractors receiving four or more negative responses to yes or no questions combined with ratings of "Needs Improvement" or "Unacceptable" will be documented as demonstrating poor performance and given two weeks to correct deficiencies. Failure to resolve issues within this timeframe may result in contract termination with thirty days' notice. Three consecutive poor performance evaluations trigger automatic termination eligibility. Contractors have fifteen calendar days to appeal termination decisions to the Director of the Division of Procurement Services. Additional contractual requirements include providing photographic documentation and detailed damage listings within thirty days of contract start, maintaining proper snow pile locations, completing all clearance tasks without creating drifts in restricted areas, and responding to all communications within one business day.
