About this file
The document is a Sensitive Security Information Acknowledgement Form from the City of Phoenix Aviation Department. The form is associated with a City of Phoenix solicitation for the purchase of motorcycles, with a contract term of three years and two potential option years, anticipated to commence on or about February 1, 2026. The procurement will be conducted through the City's Procurement Portal, with technical support available via email or an online support bubble. The solicitation allows for sealed offers and is conditioned upon award by the City Council and subsequent recording by the City Clerk's department.
The acknowledgement form specifically requires vendors to recognize the sensitive nature of certain documents, which contain information controlled under federal regulations (49 C.F.R. Parts 15 and 1520). Vendors must agree to handle and ultimately destroy these documents in compliance with specified guidelines, with destruction required upon completion of document review or by the contract end date. The form mandates that only persons with a requisite "need to know" can access the sensitive documents, with potential civil penalties for unauthorized disclosure. Vendors must complete the form by providing company details, authorized representative information, signature, and contact information.
