About this file

This is a Self-Performance Requirements document for an Illinois state and local contract opportunity involving the Department of Mental Health (DMH) at Chester Mental Health Center. The project involves replacing approximately 140 sprinkler heads in the basement level of the hospital's wet pipe fire suppression system. The document establishes mandatory performance criteria for the prime contractor undertaking this work.

The prime contractor is required to perform a minimum of 51% of the total contract work on-site using its own organization, defined as workers directly employed and paid by the prime contractor and equipment owned or rented by the prime contractor with or without operators. Verification of the prime's performance percentage will be conducted through the Illinois Department of Labor's Certified Payroll Report and visitor logs from the facility. Failure to meet the specified self-performance percentage at the time of bid submission, or any post-award deviation without prior written approval from the Owner, constitutes a material breach of contract and grounds for immediate termination for cause.
