FOR

ASBESTOS REMOVAL

STATEMENT OF WORK

FOR

ASBESTOS REMOVAL

1.0. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment and labor necessary to ensure that asbestos services performed at Hill AFB, shall be in accordance with applicable federal, state, and local regulations, and follow the procedures outlined in the respective approved work plan submitted by the contractor.

The contractor is to remove systems furniture. The contractor is to remove carpet, carpet adhesive, floor tile, and floor tile adhesive/mastic and decontaminate the finished areas. The contractor shall be required to construct two negative air flow or negative pressure enclosures. One on the west half of room 221 and the other on the east half of room 221.

Both enclosures shall allow access to room 215 and both stairwells. Upon completion of removal activities, the contractor shall wet clean all building components from the ceiling deck, down to the floor. This shall be accomplished prior to encapsulation and aggressive air clearance sampling. Asbestos removal work plan shall be submitted by the contractor to 75 CES/CEOHA for approval, prior to starting the project. The contractor shall be responsible for submitting any required NESHAP notification to the Utah Department of Air Quality (UDAQ), 10 days prior to starting the asbestos removal phase of any project when required. A copy of the NESHAP notifications shall be sent to 75 CES/CEOHA.

1.1. SAFETY BARRIERS. All wall, ceiling, HVAC equipment, supply and return grills, exhaust fans, ceiling return plenums or associated ventilation openings and lektrievers will be sealed in an airtight manner with 6-mil polyethylene sheeting and 3” duct tape. These airtight critical barriers are to remain in place for the duration of the removal project and will not be removed until the work area has passed a final visual inspection by 75 Civil Engineering and aggressive air clearance sampling has been completed. Negative air flow/pressure enclosures will be constructed out of fire retardant 4 and 6-mil polyethylene sheeting. Prior to the disturbance of any ACM, asbestos warning signs will be posted at the decontamination module entrance into the regulated area or at any openings where unauthorized entry could be made. The contractor shall provide all construction support equipment items to control and secure the removal site and any remote storage areas, including, sanitary facilities, barriers, barricades, scaffolding, fences, traffic/parking signs/signals, and flag persons when required.

The contractor is responsible for the erection and safe application of all equipment.

1.2 WORKING HOURS.

Monday – Saturday 07:00 – 16:00. Overtime work on this job must be approved through the

Abatement IDIQ PM and/or Contracting officer prior to engaging in any overtime work.

1.3. NEGATIVE AIR FLOW/NEGATIVE PRESSURE ENCLOSURE. Prior to the disturbance of any asbestos materials, the contractor shall establish negative airflow and/or pressure inside each regulated area, through the use of HEPA air filtration device’s (AFD).

These AFD’s will be located in such a way, that will allow proper directional airflow in all locations of the contained work area and provide at least four complete air changes per hour.

When using a full negative pressure enclosure, the AFD’s will provide at a minimum -0.02” water column (W.C.), differential relative to outside pressure. Maintain a monitoring device to measure the negative pressure in the enclosure and maintain this negative pressure until each area has been released as clean by a visual inspections and aggressive air clearance sampling. If the negative pressure drops below the minimal -0.02” W.C., work will be stopped immediately; until the required negative pressure is again restored. The contractor shall construct two negative pressure enclosures for this project.

1.4. STANDARD DECONTAMINATION PROCEDURE. A three-stage decontamination module will be used for entry to and exit from each work area when required. This shall consist of 1) clean change room; 2) shower unit with hot and cold running water and 3) dirty or equipment room. A double flap polyethylene barrier shall be used as a barrier between each of these rooms, as well as at the outer clean room entrance and the threshold between the equipment room and work area. The contractor’s employees/visitors shall exit through this decontamination unit only. Protective clothing shall be removed inside the dirty chamber while still wearing respiratory protection. Personnel shall rinse off in the shower first, before doffing their respirators. Upon completion of shower, personnel shall enter the clean room to dry, dress, and visually inspect respirator for damage prior to storing in proper container.

Shower and waste water shall be filtered down to 5 microns, before water is discharged into a sanitary sewer drain.

1.5. REMOVE NON-ASBESTOS CARPET. The contractor shall remove carpet from all areas not to include the north conference room and the west center cubical area. The carpet shall be disposed in the correct manner. Non contaminated carpet may be disposed of as regular construction debris. Asbestos contaminated carpet will be disposed of as ACM waste in accordance with the Utah Division of Air Quality regulation R307-801-14

1.6. REMOVAL OF ASBESTOS MATERIALS (ACM). The contractor shall remove ACM in accordance with 29 CFR 1926.1101 and as outlined in DAQ R307-801, prior to set-up and removal of any asbestos containing materials. Critical barriers will cover all openings and penetrations inside the regulated work area and will remain in place until the areas have been released as clean, first by a visual inspection and then by aggressive air clearance sampling. A government representative, certified as an asbestos inspector from 75 Civil Engineering will provide this visual inspection. An independent, third party consulting firm will run the aggressive air clearance samples. A three stage decontamination unit shall be attached to each enclosure/phase prior to starting any asbestos removal. The decontamination unit will be equipped with hot and cold running water. Shower and or residual water will be filtered down to 5 microns before being discharged into a sanitary sewer drain. Regulated area will be demarcated with asbestos warning signs and barrier tape. The contractor will remove and properly package the ACM while adequately wet.

1.7. REMOVE ASBESTOS FLOOR TILE AND MASTIC. The contractor shall remove all carpet adhesive, floor tile, black mastic, and ACM waste in Accordance with the Utah Division of Air Quality regulation R307-801-13.

1.8. CLEANING AND WASTE CONTROL The contractor shall ensure that the regulated work area is cleared of visible asbestos containing materials/debris, prior to 75 CES/CEOHA certified asbestos inspectors provide the final visual inspection. Once the contractor has passed the final visual inspection, the contractor shall lightly encapsulate the entire work area with Fosters 32-22. Dry sweep or compressed air shall not be used for cleanup. Polyethylene sheeting shall be treated as asbestos containing materials. All ACM materials shall be properly packaged while adequately wet, labeled, and disposed of in an EPA approved landfill.

1.9. AIR SAMPLE MONITORING. The contractor shall monitor all areas for potential hazards and ensure that health and safety precautions are taken during asbestos removal activities. This is to include personal breathing zone monitors as required by OSHA, area air samples, and aggressive air clearance samples when required. Personal breathing zone air sampling shall be conducted daily for each activity/job being performed inside the containment. In situations where all workers are involved in the same activity/job, the workers to be sampled will be selected based on the highest potential for exposure. These samples will then be considered as representative of all workers performing the same activity.

Thirty-minute excursion sampling will be conducted during this same, worst case activity.

Aggressive air clearance samples will be conducted by an independent, third party consulting firm. This independent firm must be licensed and certified as a consultant/inspector with the Utah Division of Air Quality. All air sample results shall be posted on site as soon as possible.

If the contractor fails the air clearance criteria, then the contractor shall be responsible for re-cleaning the regulated area and having the independent consulting firm run the aggressive air clearance samples again, at no expense to the government.

1.10. USE OF PERSONAL PROTECTIVE EQUIPMENT (PPE).

The contractor shall comply with all OSHA and AFI regulations for personal protective equipment in table 1 of 29 CFR 1926.1101

1.11. ELECTRICAL SAFETY. Ground Fault Circuit Interrupter (GFCI) and wet environment electrical cords shall be used inside the regulated work area. GFCI’s used are designed to remove power from equipment loads when these loads have a potentially lethal ground current in excess of six milliamperes. Task level lighting of at least 50 foot-candles shall be provided in the work area when necessary.

2.0. SERVICE DELIVERY SUMMARY.

Materials; Materials shall be removed with no visible dust present. Ensure safety barriers are airtight and secure at all times. All ACM is properly packaged and labeled for disposal. Work area is properly demarcated. All waste is disposed of properly at an EPA approved landfill or recycled at a qualified recycler.

1.1. - 1.11. No deficiencies permitted.

3.0. GENERAL INFORMATION.

3.1. QUALITY CONTROL. The contractor shall develop and maintain a work plan to ensure services are performed in accordance with commonly accepted commercial practices for removal of asbestos and lead based paint. The work plan shall be submitted to 75 CES/CEOHA for review. Work plan shall be approved before work begins. The contractor shall develop and implement procedures to identify and prevent defective services from reoccurring.

3.2. QUALITY ASSURANCE. The government will periodically evaluate the contractor’s performance in accordance with the Quality Assurance Surveillance Plan.

3.4. DOCUMENTATION. The contractor shall provide copies of all project documentation to

75 CES/CEOHA to include daily logs, sample results, and waste manifests upon completion of the project.

3.5. SECURITY REQUIREMENTS. Must have security clearance badges to enter onto Hill Air Force Base and a security line badge to enter onto the airfield/runway area.

3.6. SPECIAL QUALIFICATIONS. All employees of the removal contractor, must be certified to remove asbestos and accredited with the Utah Division of Air Quality (DAQ). State cards issued to the employees must be on site at all times. Contractor shall have a supervisor certified with DAQ as asbestos contractor/supervisor on site at all times.

4. CONTRACT INFORMATION. Only the contracting officer can change the scope of this task assignment. Any work performed by the contractor outside of this scope of work, without written authorization from the contracting officer shall be at the contractor’s expense with no recourse against the government or its agents. Requests for changes to this scope of work must be submitted in writing to the contracting officer. No work on the requested change will be allowed until the contractor receives a signed modification to the task assignment.

5. Performance period for this project is 60 days.
