I. Proposal Format
A. General: The following instructions cover the preparation and submittal of the offeror’s proposal for this solicitation. Offerors must follow the instructions contained herein. Offerors are cautioned that any noncompliance with the terms and conditions of the RFP may cause their proposal to be determined not eligible for award. If a joint venture arrangement exists for this acquisition, the offeror shall provide a copy of the joint venture agreement that is signed and dated by all joint venture members as part of the proposal submission. Offerors shall submit only one proposal for the Maternity Flight Duty Uniform (MFDU) as the Government will review only one proposal per offeror.
Per AFFARS MP5332.7 Contract Funding, Notice to Offeror(s)/Supplier(s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.
B. Point of Contact: The Contracting Officer (CO), Lila Ivanovska, is the primary point of contact for this acquisition. The Contract Specialist, Benjamin Gatson, is the secondary point of contact for this acquisition. Address any questions or concerns you may have to the CO and Contract Specialist. Written requests for clarification shall be sent by email to lila.ivanovska@us.af.mil and benjamin.gatson@us.af.mil. Offerors shall submit any questions relating to the RFP within 10 calendar days of the RFP release date. Answers to such clarifications will be posted as questions and answers (Q&A) on the BETA.SAM website (https://www.beta.sam.gov). In the event of conflict between any provided clarification and the RFP, the RFP shall take precedence.
Proposals must be received by 4:00pm (Eastern Daylight Time) on XXXX 2021 and be delivered electronically to:
Procuring Contracting Officer (PCO) Ms. Lila Ivanovska Contracting Specialist (CS), Mr. Benjamin Gatson

C. Debriefings: The CO will promptly notify offerors of any decision to exclude them from the competitive range, whereupon they may request and receive a debriefing in accordance with FAR 15.505. Offerors excluded from the competitive range may request a pre-award debriefing, or they may choose to wait until after the source selection decision to request a post-award debriefing (see FAR 15.505 or 15.506 as applicable). However, offerors excluded from the competitive range are entitled to no more than one debriefing. The CO will notify unsuccessful offerors in the competitive range of the source selection decision in accordance with FAR
15.506. Upon such notification, unsuccessful offerors may request and receive a debriefing.
D. Communications: Exchanges of source selection information after submission of the initial proposal packages between the Government and offerors will be controlled by the Contracting Officer. Email may be used to transmit such information only if the email can be sent encrypted and must include “Source Selection Information – See FAR 2.101 and 3.104” in the subject line of the email. In order to facilitate the sending and receiving of encrypted emails, offerors must use MS Outlook email configured to support encryption or a different email product that is S/MIME compatible and configured to support encryption. If you intend to submit your source selection information via encrypted email, you will need to contact the Buyer/Contracting Officer indicated on the face page of the solicitation prior to that first submittal in order to exchange certificates used for encryption. To ensure the process is working correctly, send a test encrypted message first (without including any source selection information). Otherwise, source selection information will be transmitted via AMRDEC SAFE at https://safe.amrdec.army.mil/safe/guide.aspx. When submitting source selection information via email, file suppression utilities, to include but not limited to zip files, will not be accepted.
E. Discrepancies: If an offeror believes that the requirements in these instructions contain an error or omission, or are otherwise unsound, the offeror shall immediately notify the CO in writing with supporting rationale. The offeror is reminded that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion.
F. System for Award Management (SAM): Offerors will not be eligible to receive a contract award from a DoD activity unless they are registered in SAM. Firms must register on a one-time basis and annually confirm accuracy and completeness of registration information. On-line registration and instructions can be accessed at https://www.sam.gov.
G. Data Universal Numbering System (DUMS) Number: A DUNS Number applies to offers exceeding $25,000. The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation “DUNS” followed by the DUNS number that identifies the offeror’s name and address. If the offeror does not have a DUNS number, Dun and Bradstreet should be contacted to obtain one at no charge. An offeror within the United States may call 1- 800-333-0505. The offeror may obtain more information regarding the DUNS number, including locations of local Dun and Bradstreet Information Services offices for offerors located outside the United States, from the internet homepage at http://www.dnb.com/. If an offeror is unable to locate a local service center, an email may be sent to Dun and Bradstreet at custserv@dnb.com.
II. Volume Organization

Offerors are cautioned that any pages that exceed the page limitations shall not be read.
4. Page limitations may be placed on responses to Evaluation Notices (ENs), if issued. The specified page limits for EN responses will be identified in the letters forwarding the ENs to offerors or on the EN form itself.
5. Each page containing proprietary information should be so marked.
6. Each page should contain the following legend at the bottom of each sheet:
SOURCE SELECTION INFORMATION--SEE FAR 2.101 and 3.104
FOR OFFICIAL USE ONLY
7. RFP shall consist of the completed and signed RFP, to include any amendments issued, with a cover letter delineating any exceptions taken to the RFP terms and conditions with accompanying rationale. However, offerors are cautioned that any noncompliance with the terms and conditions of the RFP may cause their proposal to be determined not eligible for award. Offerors shall ensure that all clauses and provisions that require “fill-in” information are appropriately completed, including the proposed prices associated with the contract line items in the Schedule of the RFP.
B. Volume I, Executive Summary In the executive summary volume, the offeror shall provide the following information:
1. Narrative Summary: The narrative summary of the entire proposal should be concise, to include addressing the significant risks, and highlighting any key or unique features, excluding price. The salient features should tie in with Section M evaluation factors/subfactors. Any summary material presented here shall not be considered as meeting the requirements for any portions of other volumes of the proposal.
2. Table of Contents: Include a master table of contents of the entire proposal.
3. Cross Reference Matrix: The offeror shall create and submit a cross-reference matrix indicating where in its proposal the information can be found as it relates to the RFP.
The cross-reference matrix will be utilized as a tool to show critical interrelationships and dependencies among the technical requirements document (Statement of Work (SOW), Fielding Plan, Purchase Description (PD), the Contract Data Requirements List (CDRL), Section L (Instructions to Offerors) and Section M (Evaluation Basis for Award)). The cross-reference matrix will help an offeror ensure it has responded to all the evaluation criteria and proposal submittal requirements identified in the solicitation. If the matrix conflicts with any other requirement, direction, or provision of this solicitation, the other reference shall take precedence over the matrix. Additionally, to the extent the matrix discloses details as to the manner by which the Government intends to evaluate the offeror’s proposals for award, Section M references in the matrix are for informational purposes only, and the Government shall be obligated to evaluate proposals solely in conformance with Section M of the solicitation.
C. Volume II, Written Technical Proposal
1. General: The written technical proposal shall be clear, concise, and include all the information required by this provision in sufficient detail for effective evaluation. Your responses will be evaluated against the Technical subfactors defined in Section M, Evaluation Factors for Award. Using the instructions provided below, provide as specifically as possible the actual methodology you would use for accomplishing/satisfying these subfactors. All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that your firm will perform all the requirements specified in the solicitation. It is not necessary or desirable for you to tell us so in your proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation. The proposal should not simply rephrase or restate the Government’s requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume the Government has no prior knowledge of their capabilities, work processes, facilities, and experience and will base its evaluation on the information presented in the offeror’s technical proposal.
2. Format and Specific Content: The following information shall be provided in the written technical proposal and will be evaluated to assess the proposed technical approach and associated technical risk in accordance with Section M, Evaluation Basis for Award. The written technical proposal shall address each of the following subfactors to describe the offeror’s proposed approach to performing the requirements set forth in the SOW/PD/Fielding Plan, CDRLs. For each subfactor, the offeror shall identify risks, if any, associated with the proposed approach and actions the offeror will take to mitigate the identified risks. If no risks/mitigations are identified in the offeror’s proposal, it indicates the offeror does not consider there to be any risk associated with their proposed approach.
3. Subfactor One: Product Demonstration Model (PDM)
1) Description: This subfactor evaluates the vendor’s ability to perform the
Government’s key capabilities in accordance with the Purchase Description (Attachment 2).
2) A responsive proposal shall include:
a) Offerors will be required to submit one (1) size Medium-Regular PDM for each production facility/place of performance. The offeror shall submit PDM(s) as part of its proposal. PDMs will be submitted at no expense to the Government and must be received prior to the time and date set forth for closing of offers. PDMs will be returned at the offeror's request and expense so long as the PDM(s) is(are) not destroyed by testing, retained by the Government as a manufacturing standard, and/or unless otherwise specified by the solicitation. PDMs will become Government property unless return is requested within 30 days after date of award. The PDMs will be evaluated for conformance to each item's specification characteristics. Failure to furnish PDMs by the time and date specified in the solicitation may be cause for rejection of the entire offer if not otherwise acceptable under the provisions for considering late offers.
b) It is preferred that the materials used to manufacture the PDM be in accordance with any specifications defined in the Item Description of this solicitation. However, if the offeror uses alternate materials to manufacture the PDM(s) a letter must be supplied with them stating (i) which materials depart from the end item specification and (ii) that, although the materials used in producing the PDM(s) differ from those specified, the materials required by the specification will be utilized under any resultant contract.
c) The use of alternate materials will not affect the rating of the PDM, either favorably or negatively when the use of the alternate materials is identified in accordance with the requirements of this section. However, the Government reserves the right to give a negative rating to PDM(s) that is(are) made with materials that depart from the end item specification and the use of those alternate materials was not identified in accordance with the requirements of this section. Accordingly, to preclude a negative assessment of a PDM that is comprised of materials that are not in accordance with the specification(s), it is in the offeror's best interest to identify any and all alternate materials that have been used.
d) Although the offeror is permitted to use alternate materials to construct the
PDM(s), the offeror is not permitted to utilize alternate manufacturing operations or change the construction/design of the PDM. The PDM shall be manufactured (i) at the same facility as the production quantity and (ii) must be in accordance with applicable specifications, including the placement of the labels, unless otherwise identified.
e) A PDM is required from each place of performance/manufacture on the subject offer. The offeror shall produce both the PDM(s) and the production quantity at the same facility and certify to this effect by submission of the PDM(s). The offerors shall list the place of manufacturer under the Place of Performance Clause within the solicitation. Approval of the PDMs as part of this technical proposal does not relieve the potential awardee from the requirements of the solicitation. Manufacturing operations for the placement of labels must be completed to indicate compliance with the specification description. The labels need not be printed for the purpose of the PDM(S);
however, the end item size must be indicated. PDM(s) shall only be furnished with the offeror's proposal at the time and date of the initial closing of the solicitation. Subsequent submissions will not be permitted, even if deficiencies exist in the PDM(s) submitted. Therefore, the offeror must furnish its best effort at this time. Corrections to the PDM(s) will not be accepted by the Government.
4. Subfactor Two: Production Capability:
a) Description: This subfactor evaluates the vendor’s production capacity & quality, to include the ability to manufacture and deliver 2,000 MFDUs.
b) A responsive proposal shall include: A completed Pre-Award Survey, which specifies how the vendor meets the requirements of the MFDU.
5. Subfactor three: Demonstrated Prior Experience:
a) Description: Documentation and explanation of the contractor’s products, previous uniform efforts and evidence of contract performance which demonstrates the vendor can successfully perform. Contract performance documentation should be similar in scope and magnitude to this effort.
b) A responsive proposal shall include: Documentation of contract performance for three (3) or more contracts that are similar to the requirement stated in the SOW & Purchase Description.
The contracts provided shall be from the last five (5) years (the five-year period preceding the initial closing date of the solicitation). The government will contact the offeror’s points of contact, if needed, to verify the offeror’s performance.
D. Volume III, Price
1. Price Reasonableness: This section is to assist you in submitting data other than certified cost or pricing data that is required to evaluate the reasonableness of your proposed price. Compliance with these requirements is mandatory and failure to comply may result in rejection of your proposal.
2. Price Matrix: Offerors shall provide a completed Pricing Matrix in accordance with the specific instructions included in the spreadsheet (Attachment 3). The completed spreadsheet shall be submitted as an electronic Microsoft Excel file. The CLIN structure and price matrices represent the required flexibility and scalability needed for this contract. Failure to provide all prices will render an initial proposal submission as incomplete. The completed pricing matrices will become part of the awarded contract in Section J.
3. Price Data: Data beyond that required by this instruction shall not be submitted, unless you consider it essential to document or support your price position. All data relating to the proposed price, including all required supporting documentation, must be included in the section of the proposal designated as the Price Volume. Under no circumstances shall this data and documentation be included elsewhere in the proposal.
4. Price Data Requirements: The CO has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the CO will review this determination and if, in the CO’s opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. IAW FAR 15.403-1(b) and 15.403-3(a), information other than cost or pricing data may be required to support price reasonableness. Information shall be provided IAW FAR 15.403-5. If, after receipt of proposals, the CO determines that there is insufficient information available to determine price reasonableness and none of the exceptions in FAR 15.403-1 apply, the Offeror shall be required to submit cost or pricing data.
5. Rounding: All dollar amounts provided shall be rounded to the nearest dollar (any remainder equal to or over $0.50 shall be rounded up, under $0.50 shall be rounded down).
E. Volume IV – Contract Documentation
1. Model Contract/Representations and Certifications The purpose of this volume is to provide information to the Government for preparing the contract document and supporting file. The Offeror's proposal shall include a signed copy of the Model Contract, Sections A through K.

Complete blocks 12, 13, 14, 15, and 16. Sign and date (blocks 17 and 18 respectively) the Standard Form 33 (SF 33). Signature by the Offeror on the SF 33 constitutes an offer, which the Government may accept. The "original" copy should be clearly marked under a separate cover.

No notes.
5. Section I - Contract Clauses

Offerors must clearly identify any exception to the solicitation’s terms and conditions and provide complete accompanying rationale.
Each exception shall be specifically related to each paragraph and/or specific part of the solicitation to which the exception is taken. Provide rationale in support of the exception and fully explain its impact, if any, on the performance, schedule, cost, and specific requirements of the solicitation.
