About this file
The document is a Supplier Diversity Program (SDP) Spending Report form for a state and local contract opportunity. The Department of Children and Families (DCF) in Massachusetts is seeking proposals from qualified bidders to conduct culturally and linguistically competent and impartial parenting capacity evaluations. Approved bidders will form a pool of provider resources across the Commonwealth available to conduct parenting capacity evaluations in response to referrals submitted by DCF's 29 Area Offices. Prime contractors are required to complete this quarterly spending report to document their spending with certified Supplier Diversity Program (SDP) partners. The report includes information on the prime contractor's annual SDP spending commitment, quarterly contract sales, and detailed SDP partner spending by quarter.
The report requires the prime contractor to provide information on their SDP partners, including the partner's name, certification type, and relationship type (subcontracting or ancillary). Prime contractors must also report the quarterly and annual spending amounts with each SDP partner. The form includes instructions for accurately completing each section and provides links to resources for identifying eligible SDP partners.
