DEPARTMENT OF HOMELAND SECURITY

BASE HONOLULU

HONOLULU, HAWAII 96819

SCOPE FOR

WPB SUPPORT BUILDING SEWER MANHOLE REPAIR

101.1 SCOPE OF WORK: Furnish all labor, materials, equipment, and professional service's necessary to conduct the following for Base Honolulu:

A. BASE ITEMS: Work includes but is not limited to:

1) Design and construct manhole benching and invert channels for WPB building manhole.

2) Inverts must be true to the sewer pipe invert elevation with smooth channels of uniform cross section and slop, either straight or with a continuous curve between the inlet and outlet of the pipes. Concrete inverts should be placed based on the dimension and details of the contract plans and specification.

3) Contractor shall follow OSHA section 1926.953 safety requirements for confined space entry before a manhole cover can be removed.

4) The Contractor shall perform, and atmospheric pressure build up test, temperature build up test an<l or hazar<lons atmosphere test anci a pl,m to mitig<lfe these h,mmls

5) Contractor shall install a temporary railing, cover or other barrier erected around the manhole immediately after the manhole is safely removed.

6) Contractor shall have a confined space tripod rescue system for safety entry, exit, rescue an or retrieval of personnel.

7) Contractor shall provide fall protection and rescue, continue gas detection, and possible force air ventilation.

8) Work shall be performed by or under the direct supervision of journeyman level workers in their respective trades.

101.2 LOCATION: Coast Guard Base Honolulu: 400 Sand Island Pkwy, Honolulu Hi. 96819, WPB

Support Building.

101.3 LIASON: The Project Manager (PM) {TBD) by Base Engineering.

101.4 PERIOND OF PERFORMANCE:

101.5 CONTRACTOR SECURITY REQUIREMENT: The contractor shall provide a list of all onsite personnel, including sub-contractors (including second and third-tier subcontractors), to the PM prior to any site visit. Contractor personnel not listed may be denied access to the Coast

Guard facility. Contractor personnel will be restricted to designated working areas. Any personnel violating this policy may lose access to the Coast Guard facility. Contractor personnel shall always have photo identification while working on Coast Guard facilities. All contractors, including second and third-tier subcontractors, shall have active Rapid Gate passes for

USCG Base Honolulu prior to the start of work.

101.6 CONTINUITY OF FACILITIES OPERATION: Schedule work to minimize interference with the facility’s normal operations. Notify the Contracting Officer and the site POC 10 days in advance of any shutdowns. Perform all on-site work between the hours of 7:00 AM and 3:00 PM, Monday through Friday (Federal Holidays excluded), unless otherwise approved by the PM.

101.7 SAFETY: During the execution of this contract, the Contractor shall conform to the rules and regulations as set forth by OSHA Safety and Health Standards, 29 CFR Part 1926 — Safety and Health Regulations.

101.8 TEMPORARY FACILITIES:

A. SANITARY FACILITIES: Toilet facilities are available for Contractors use at or near the job site. The Contractor shall ensure the facilities remain clean or permission shall be revoked

B. WATER AND ELECTRICITY: Water and 120V electricity are available at no cost to the Contractor. Cost of these utilities connections and disconnection shall be the responsibility of the Contractor.

C. TELEPHONE: Not available at site.

D. GENERAL INFORMATION:

1) Contractor’s Storage Area.

(a) Storage of material and equipment is responsibility of Contractor.

(b) Coordinate storage area with contracting officer representative (COR).

(c) Contractor has full responsibility for security of all stored materials and equipment.

2) Permits and Notices. Notify Contraction Officer Representative (COR) and Contracting Officer 10 working days in advance of any of the following planned actions.

(a) Commencement or recommencement of work at the site.

(b) Shutdowns (Electrical, Water, HVAC, etc.)

(c) Request for inspection of critical job elements.

(d) Testing.

101.9 SITE VISIT: Bidder are encouraged to visit the site to veril5’ existing conditions and to determine the extent of the work. Contact the U.S. Coast Guard Base Honolulu Facilities Engineering to arrange for a site visit.

102.1 WORKING DIRCTIVES:

• It is the Contractor’s responsibility to cover surfaces that may be damaged during the work.

Any damage as result of the Contractor ‘s negligence will remain the Contractor’s to correct.

• Litter, debris, and removal of construction materials shall be removed from the work site at the end of each workday. Health and safety cannot be stressed enough.

• The Contractor shall work from 7:00 am to 3:30 pm Monday through Friday with no work occurring on Federal holidays or weekends. Any deviation from the work schedule must have prior approval of the Contracting Officer.

• The Contractor is responsible to obtain all the necessary permits.

• All personnel employed on site shall keep within the limits of the work.

• Contractor’s equipment shall be conspicuously marked for identification.

• All litter and debris shall be cleaned and removed from the construction site at the end of each workday. Health and Safety cannot be stressed enough.

SPECIAL CONDITIONS

102.2 SCOPE:

This section contains pertinent information concerning work schedules, station regulations, and safety.

102.3 STATION AND LOCAL REGULATIONS:

• The Contractor shall comply with all station and local regulations concerning personal conduct, security, and safety.

• All personnel employed on site shall keep withing the limits of the work and shall not enter any restricted area unless required. The Contractor’s equipment shall be conspicuously marked for identification.

102.4 TRAFFICE CONTROL AND PARKING:

A. The Contractor shall be responsible to provide traffic control whenever work or equipment encroaches into the roadways.

B. It shall be the responsibility of the Contractor to control his employee’s traffic on the site.

Parking for all workers shall be in suitable locations as directed by the Contracting Officer’s Representative (COR).

C. Prior to the start of the project, the Contractor shall submit a list of all personnel expected to be employed on the site, (including full name, social security number, home address, date of birth) to the Contracting Officer.

102.5 SECURITY

The Contractor shall be responsible for security of his material, tools, and equipment. The Government shall not be responsible for loss or damage.

102.6 COORDINATION

The Contractor shall coordinate all work with the Contracting Officer Representative (COR) and the Facilities Engineering Office to minimize disruption in the daily operations to the Station.
