About this file
This is an Insurance Requirements document for the City of Meriden, Connecticut, establishing mandatory insurance coverage terms for a consultant contracted to provide air duct cleaning services at municipal facilities including the Police Station and Fire Stations 1-6. The project involves removal of accumulated dust, debris, and contaminants from HVAC air distribution systems across multiple city locations to improve indoor air quality and system efficiency. Services must be performed with minimal disruption to police and fire station operations while maintaining safe and functional environments for city personnel. The consultant must procure and maintain four categories of insurance coverage for the life of the contract: Commercial General Liability Coverage with limits of $1,000,000 per occurrence and $2,000,000 aggregate, including premises/operations, products-completed operations, underground/explosion/collapse hazards, contractual liability, independent contractors, and abuse and molestation liability; Business Automobile Liability Coverage with $1,000,000 combined single limit covering non-owned, all owned, and any automobiles; State of Connecticut Statutory Workers' Compensation and Employer's Liability Coverage with limits of $500,000 per accident, $500,000 disease policy limit, and $500,000 per employee disease; and Professional Liability, Malpractice, or Errors and Omissions Coverage with $2,000,000 single limit per claim and $2,000,000 aggregate per policy period, which may be written on an occurrence or claims-made basis.
