About this file
This document is a collection of administrative and security forms issued by the Michigan Department of Technology, Management and Budget (DTMB) for contractors and vendors seeking access to the State of Michigan's systems and facilities. The package includes five forms designed to facilitate secure onboarding of external personnel: the Background Check Request (DTMB-0021), Contractor Security Agreement (DTMB-0928), Foreign Device Network Connection Request (DTMB-0155), Michigan Cyber Security Vendor SecurID Questions (DTMB-0051), and Non-Disclosure Agreement (DTMB-0049). These forms establish the procedural and security requirements contractors must complete prior to gaining access to state inventory systems, remote access services, or state facilities. No specific project scope, quantities, response deadlines, or contract award dates are indicated in this document.
The forms collectively establish security protocols and compliance obligations for contractor personnel without establishing pricing terms, set-asides, or funding amounts. Contractors must submit background check information, sign security agreements acknowledging compliance with Michigan Computer Crime Law and information security policies, complete equipment security assessments for foreign devices, and execute non-disclosure agreements protecting confidential and proprietary state information. Requirements vary based on employment type, scope of access, and equipment usage; for example, background checks must be current within one year, and vendors using state-owned computers are exempt from foreign device network connection requests. The Non-Disclosure Agreement establishes protections for state confidential information, trade secrets, and personally identifiable information, with survival provisions extending confidentiality obligations beyond agreement termination.
