About this file
This document is a Summary of Benefits for the Medicare Plus Blue Group PPO 2024 plan offered to members of the Michigan Public School Employees' Retirement System (MPSERS). The plan is a Medicare Advantage Preferred Provider Organization (PPO) effective January 1, 2024, through December 31, 2024, and covers eligible retirees who meet Medicare eligibility requirements and reside within the service area, which includes all 50 states and U.S. territories. The plan encompasses comprehensive medical, behavioral health, diagnostic, prescription drug, preventive care, rehabilitation, and specialty services delivered through both in-network and out-of-network providers, with Blue Cross providing network administration and customer service.
The plan establishes an annual deductible of $800, an out-of-pocket maximum of $1,700, a coinsurance maximum of $900, and a $2.5 million lifetime maximum for services not covered by Original Medicare, with an additional $1,000 annual restoration provision. Most covered services require members to pay 10 percent coinsurance with deductible application, while emergency room visits cost $135 and urgently needed services cost $65. Preventive care services, including screenings, vaccinations, and wellness visits, are covered at 100 percent with no cost sharing. The plan includes specialized benefits such as hearing aids through TruHearing (copays ranging from $499 to $1,598 per pair), SilverSneakers fitness programs at no cost, home health services at no cost, hospice through Medicare-certified providers, and worldwide coverage for hospital and medical benefits at the same cost-sharing rates as domestic services. Members must continue paying their Medicare Part B premiums and any plan premium contributions defined by the Michigan Office of Retirement Services.
