Section J Attachment B

SAQMMA17R0131

Request for past performance information for Thank you for assisting our procurement effort by providing us information on your experience with this contractor. This Past Performance Information Request focuses on [Offeror’s] experience in transitioning successfully from one contract/contractor to another, and in system installations, training, help desk support, and configuration management.

Both performance risk and quality of past performance will be rated. Performance risk will be assessed in terms of the relevance of the Offeror’s referenced experience in the areas of the contract noted above. Quality of past performance focuses on quality of service delivery, timeliness and responsiveness of performance, effectiveness of program management, and cost control. We have organized our questions to reflect these two areas, preceded by a section requesting general contractual information.

We are providing a guide at the beginning of Section II.B of this questionnaire as to the meaning of each of the adjectives listed for you to use in rating Offeror performance. This will assist you in completing the attached questions in a manner similar to that of the other references who are also being asked to complete this same questionnaire. Based on these descriptions, please circle the letter of the rating that most closely corresponds to your experience with this Offeror.

Please note: Our use of the word “Offeror” in the questionnaire below is in the context of this procurement. In regard to the efforts for your organization under the referenced contract, please interpret “Offeror” as the Contractor or Subcontractor responsible for these efforts.

When you have completed this questionnaire, please return it by Fax to 703-875-6699 or e-mail it to stewartj@state.gov.

Thank you again for your assistance.
