Factor/Rating Acceptable Unacceptable Neutral Technical Specifications
All items proposed for "exact match" line items are the same manufacturer, brand, and part number as required by the RFP.
All items proposed for "Brand Name or Equal" line items meet the salient physical, functional, and performance characteristics specified in the RFP.
One or more items proposed for "exact match" line items are not the same manufacturer, brand, or part number required by the RFP, or one or more items proposed for "Brand Name or Equal" line items do not meet the salient physical, functional, and performance characteristics specified in the RFP.
Past Performance Based on the offeror's recent (within 3 years), relevant past performance history, the Government has confidence the offeror can successfully perform the requirements in the Statement of Work.
Based on the offeror's recent (within 3 years), relevant past performance history, the Government has no confidence the offeror can successfully perform the requirements in the Statement of Work. Any documented instances of poor past performance in the Contractor Performance Assessment Reporting System (CPARS) or any Termination for Cause/Default in the past (12) months will result in an unacceptable past performance rating.
In the case of an offeror without a record of relevent past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance and will be given a neutral rating.

Performance Incentive Plan
The offeror proposes a performance incentive plan that includes all necessary performance metrics and service level agreements that are meaningful, measurable, and controllable. The proposed plan adequately explains how performance metrics and incentives will ensure and enhance both operational performance and achievement of Government objectives and business results, using adequate service level targets that are consistent with both cost effectiveness and the offeror's service delivery approach. The plan clearly explains how performance metrics and incentives will be tracked, calculated, and communicated to the Government. The plan proposes incentives and disincentives that are commensurate with stated performance goals and service levels, and has few weaknesses and no significant weaknesses or deficiencies.
The performance incentive plan does not include all necessary performance metrics and service level agreements or proposed metrics and service level agreements are not meaningful, measurable, or controllable. The proposed plan does not adequately explain how performance metrics and incentives will ensure and enhance operational performance or achievement of Government objectives and business results, or service level targets are inconsistent with cost effectiveness or the offeror's service delivery approach. The plan does not clearly explain how performance metrics and incentives will be tracked, calculated, or communicated to the Government. The plan proposes incentives and disincentives that are not commensurate with stated performance goals and service levels or are inappropriate for services under this contract. The proposed performance incentive plan has major weaknesses or deficiencies that cannot be corrected without major changes to the proposal, and substantial or extreme doubt exists that the offeror is capable of adequately quantifying, measuring, tracking, and reporting applicable performance measures.
