About this file
This document is a Sample Change Order template from the Pinellas County Purchasing Department in Clearwater, Florida, designed to formally document modifications to an existing contract. The form provides a standardized framework for recording changes to the total agreement amount, including potential additions or deletions, with designated spaces for fiscal number, change order number, and account number. The template includes signature lines for various county officials, including the Director of Purchasing, County Attorney, Board of County Commissioners Chairman, and the Clerk, as well as space for the contractor's company representative.
The change order includes a critical clause stating that the contractor's execution of the document constitutes a waiver of additional time or compensation claims for activities prior to the change order's execution. It explicitly notes that the authorized payment represents full compensation for labor, materials, incidental expenses, overhead, profit, impact costs, and associated work time. While this specific document is a sample template and not a completed change order, it is likely associated with Pinellas County's FY 2024 Pavement Preservation Package 3, which involves milling and resurfacing approximately 36.05 miles of roadway across four paving locations, including one arterial and three neighborhood projects.
